Objective:The purpose of this study was to investigate the clinical effect of transanal balloontype tracheal intubation after anterior resection of low rectal cancer to prevent anastomotic leakage and other postoperative related complications. Methods:The clinical data of 100 cases of low rectal cancer performed TME from October 2017 to May 2019 in the First Affiliated Hospital of China Medical University were analyzed retrospectively. All the cases were performed anus-retained peration without defunctioning colostomy or ileostomy.49 patients with balloon-type tracheal tube in anal were regarded as observation group,51 patients without the tube as a control group,The incidence of postoperative anastomotic leakage?postoperative hospital stay and other complications were compared between the two groups. Results:There were no significant differences in clinical data between the test group and the control group on patient demographics, preoperative complications,BMI,preoperative albumin, operation time, and surgical methods(P>O.05).Total incidence of postoperative anastomotic leakage is 12.00%,The incidence of anastomotic leakage in the observation group was 4.08%(2/49).All patients were cured and discharged after conservative treatment,Anastomotic leakage occurred in 10 of the 51 patients in the control group, with an incidence of 19.61%.Five of them underwent proximal colostomy and were discharged after treatment. The incidence of anastomotic leakage was lower in the observation group than in the control group, and the difference was statistically significant. The second surgery rate of patients with anastomotic leakage in the observation group was lower than that in the control group, but the difference was not statistically significant(P = 0.47),the postoperative hospital stay in the observation group was shorter than that in the control group [(12.35±5.17)d vs(16.29±12.96)d,P=0.048].in the incidence of postoperative bleeding[0.00%(0/49)vs2.00%(1/51)P=1.00]?postoperative diarrhea [4.08%(2/49)vs 2.00%(1/51)P = 0.61],early postoperative intestinal obstruction [4.08%(2/49)vs 5.88% P = 0.61] in two groups of patients in the two groups of patients were no significant differences. All 12 patients with anastomotic leakage were cured and discharged after treatment, and none died during hospitalization. Conclusions:After anterior resection of low rectal cancer, transanal balloon tracheal intubation decompression can effectively reduce the occurrence of postoperative anastomotic leakage.
