Object: To investigate the feasibility of those parameters measured with conventional ultrasound and shear wave elastography(SWE)in monitoring the efficiency of transjugular intrahepatic portosystemic shunt(TIPS).Method: From December 2017 to December 2019,21 patients who underwent TIPS in the interventional department of our hospital and maintained stent patency during three consecutive follow-ups after surgery were analyzed.Three consecutive followups were performed within 1 month,1-3 months,and 3-6 months after TIPS.Conventional ultrasound observations include spleen thickness,splenic vein diameter(SVD),splenic vein blood flow velocity(SVV),portal vein diameter(PVD),and portal vein blood flow velocity(PVV).SWE observations include liver stiffness(LS)and spleen stiffness(SS).When the stent was proved to be unobstructed by contrastenhanced ultrasound,conventional ultrasound indicators and SWE indicators were included in the study.Statistical analysis using SPSS 25.0 software,Statistical significance was defined as P<0.05.Results: PVV within 1 month after TIPS was 32.92 ± 11.27 cm/s,and PVV within 1-3 months after TIPS was 27.14 ± 9.28 cm/s,a significant decrease compared with PVV within 1 month after TIPS(P<0.05).The PVV was 25.16 ± 7.57 cm/s within 3-6 months after TIPS,which was significantly lower than that within 1 month after TIPS(P<0.05).However,no significant difference in PVV was found between 1-3 months and 3-6 months after TIPS.SS within 1 month after TIPS was 35.77 ± 6.32 k Pa,and SS within 1-3 months after TIPS was 32.56 ± 6.82 k Pa,significantly lower than SS within 1 month after TIPS(P<0.001).SS within 3-6 months after TIPS was 30.15 ± 6.29 k Pa,lower than SS within 1 month after TIPS(P<0.001).However,there was no significant difference in SS between 1-3 months and 3-6 months after TIPS.After validating the data with the Mauchly sphericity test,repeated-measures analysis of variance was used to examine the differences in spleen thickness,PVD,PVV,SVD,SVV,LS and SS.No significant difference was found in spleen thickness,PVD,SVD,SVV and LS in the three groups of data for each indicator.Conclusion: When the stent remained unobstructed after TIPS,PVV and SS are on the decline in the short term.The tendency has certain clinical significance for the detection of stent function and postoperative follow-up.
