Objective: To observe the clinical effect and value of skin soft tissue expansion combined with autogenous costal cartilage transplantation in the treatment of congenital malformation of children.Methods: 60 cases(60 ears)of congenital microtia received by our hospital from April 2018 to April 2019 were selected for clinical analysis.Based on the retrospective analysis,30 patients were divided into control group and observation group.The basic information,operation related indexes,symmetry score,appearance score,anatomical structure and stability score,postoperative complications,quality of life score and treatment effect were observed.Results:(1)there was no statistical significance(P > 0.05)in the comparison of basic information of gender,age,side and shape between the two groups.(2)The amount of intraoperative hemorrhage was(105.34 ± 10.47)ml in the observation group and(110.39 ± 20.45)ml in the control group.There was no statistical significance between the observation group and the control group(P > 0.05).(3)The total score of symmetry of reconstructed auricle in the observation group was(35.25± 0.93)and that in the control group was(25.08 ± 1.27).The difference between the observation group and the control group was statistically significant(P < 0.05).(4)The total score of the observation group was(26.07 ± 0.63)and the control group was(16.85 ± 1.04).The difference between the observation group and the control group was statistically significant(P < 0.05).(5)The total score of reconstructed auricle in the observation group was(27.18 ± 0.57)and that in the control group was(20.49 ±0.64).The difference of anatomical structure and stability between the observation group and the control group was statistically significant(P < 0.05).(6)The total incidence of complications was 6.67% in the observation group and 26.67% in the control group.The difference of postoperative complications between the observation group and the control group was statistically significant(P < 0.05).(7)The total score of quality of life in theobservation group was 94.32 ± 1.27,and that in the control group was 80.14 ± 1.28.The difference of QOL score between the observation group and the control group was statistically significant(P < 0.05).(8)The total effective rate was 96.67% in the observation group and 73.33% in the control group.Conclusion: the application of dilatation technique combined with autologous costal cartilage transplantation in total ear reconstruction of congenital malformation in children can improve the symmetry,aesthetics and stability of reconstructed auricle,and fully reduce the incidence of postoperative complications.To improve the quality of life of patients and improve the therapeutic effect of clinical diseases,the clinical application value is relatively prominent and remarkable.The dilatation technique combined with autologous costal cartilage transplantation can be widely used in the treatment of congenital small ear malformation.
