Objective To explore the effects of t Hcy,MLR,SII and OSTA index on osteoporosis(OP)of elder women.We analyse the relationship between general clinical conditions,various metabolic indicators,inflammatory indicators and bone mineral density(BMD),and then assess the ability of the OSTA index to assess the risk of OP to provide theoretical basis and clinical guide for the early screening,diagnosis,prevention and treatment of OP in old females.Method We collect 310 elderly women who saw doctors at the physical exa mination center of the affiliated hospital of Qingdao university from July 2017 to January 2020,and each of them with the age of 60-70 years old and natural men opause.We collect general information of each participant,including age,height,weight,menopausal age,years of menopause,medical history and so on.We calcu late the osteoporosis self-assessment tool for Asians(OSTA)index based on the fo rmula.Every participant was taken fasting blood in the morning.We test the conc ent of total homocysteine(t Hcy)and determine the amount of cellular component in the blood routine,and calculate the monocyte-to-lymphocyte ratio(MLR),syste matic immune-inflammatory index(SII)according to the formula respectively.And we make detailed statistics on other biochemical indicators of each participants.Th en we use dual energy X-ray absorptiometry(DXA)to determine the BMD of eac h participants’ in T-value and divide them into osteoporosis group and non-osteopo rosis group according to WHO criteria.SPSS22.0 software is used for statistical a nalysis to compare the data of these two groups for statistical differences.At thes ame time,we drew a matrix point chart to analyze the linear relationship between relevant indicators and T-value of lumbar spine and femoral neck.Finally,we dra w the ROC curve to study the predictive value of tHcy,MLR,SII and OSTA ind ex on the risk of osteoporosis in elderly women.Result1.The tHcy and SII in the osteoporosis group is significantly higher than those in the non-osteoporosis group(all p<0.05),OSTA index is significantly lower than this index in the non-osteoporosis group(p<0.05).There is no statistically difference in MLR between two groups(p>0.05).2.Binary Logistic regression analysis shows that tHcy,SII and OSTA index are independent risk factors for osteoporosis in women aged 60-70 years(p<0.05).3.The matrix dotted figure shows that tHcy is negatively correlated with lumbar T-value or femur T-value,SII is negatively correlated with lumbar T-value or femur T-value.While OSTA index is positively correlated with lumbar T-value or femur T-value(all p<0.05).4.According to the ROC curve,the area of the ROC curve for tHcy,SII and OSTA index to identify the risk of osteoporosis in elderly women aged 60-70 years is 0.607,0.613 and 0.653 respectively.The sensitivity of t Hcy,SII and OSTA indexes to identify osteoporosis respectively is 51.1%,68.6% and 69.5%.The specificity respectively is 66.3%,57.7% and 63.2%.The AUC of the combined indicators diagnostic value is significantly higher than that of the single diagnosis of tHcy,SII or OSTA index,and the difference is statistically significant(all p<0.05).Conclusion1.In this study we can cee tHcy,SII and OSTA index are independent risk factors for osteoporosis in elderly females,and there is little correlation of MLR with osteoporosis in old women.THcy,SII OSTA index all have a predictive effect on osteoporosis in elderly women to some extent.2.The prediction value of tHcy and SII combined with OSTA index on the risk of osteoporosis in elderly women is higher,but further research is needed.3.THcy,SII,OSTA index or the combination of three indicators all could be used to assess the risk of osteoporosis in elderly women.And they are economical,convenient and easy to be promoted,but needs more studies.
