Objective:To study the effect of different methods of tourniquet application on postoperative function of total knee joint replacement patients and optimize the application of tourniquet.material and method:Materials used in October 2018 to October 2019 in our hospital(the first central hospital of baoding city)orthopaedic line unilateral TKA for the first time strictly exclusion and inclusion criteria,80 patients using EXCEL RAND program generated two groups at random,qq group(intraoperative application tourniquet group: from the beginning to cut skin suture,the entire application tourniquet),W group(only put false body when using a tourniquet,partial application tourniquet),unified management,strictly from hospital start intraoperative and postoperative strictly to collect data.Has the recessive blood loss,postoperative blood loss,postoperative total blood loss,operative time,postoperative VAS score(8 hours,after 1 day,5 days,3 days,7 days,14 days)and postoperative complications of 14 days(skin blisters,superficial infection,wound healing delay,deep vein thrombosis,etc.),the quadriceps muscle and postoperative limb muscle(said)the straight leg-raising time,postoperative knee range.The differences between the two groups were compared according to the statistics of each evaluation indexResults:The study showed that on the 1st,3rd and 5th day after the operation,the pain score of active and inactive patients in Q group was higher than that in W group(partial tourniquet group),and on the 7th day after the operation,the pain score of active and inactive patients in Q group was higher than that in W group.The intraoperative blood loss in group W was significantly higher than that in group Q,and there was no significant difference in total blood loss on the 7th day after surgery.The mean operation time of group W was longer than that of group Q.The incidence of postoperative complications in group W was significantly reduced.The active range of knee joint in Q group was less than that in W group,and the recovery of quadriceps muscle strength was slower.Conclusion:Partial application tourniquet more than the entire application tourniquet hemorrhage in group a,operation time is prolonged,no difference between the total blood loss,the entire application tourniquet increased postoperative pain,two groups of tourniquet use DVT not significantly affect the single complications,such as partial application tourniquet significantly reduced the total complications,partial application tourniquet postoperative functional recovery faster,so some applied a tourniquet is an ideal way of applying a tourniquet.
