A Study On The Serum AngⅡ、ang(1-7)and 5-HT Level In Patients With Chronic Heart Failure Complicated Anxiety Depression Disorder

Objectives To study the level and relationship of serum ang Ⅱ,ang(1-7)and 5-HT in patients with chronic heart failure and anxiety and(or)depression.Methods The patients with chronic heart failure who were treated in the affiliated hospital of North China University of Science and Technology from December 1,2018 to January 31,2020,were selected as the research subjects.During the hospitalization,they were closely followed up and observed.According to the inclusion and exclusion criteria,patients willing to cooperate were selected,164 patients were selected.Collect general data of patients;SAS and SDS scores of study subjects,According to the score,the heart failure subjects were divided into chronic heart failure with anxiety and(or)depression group,chronic heart failure without anxiety and(or)depression group;improve the collection of blood biochemical,renal function,heart color Doppler ultrasound and other related indicators of the research object;collect the fasting plasma of the observation object,measure the AngⅡ,Ang(1-7)and 5-HT index levels of the patient using ELISA kit,use SPSS17.0 Statistical analysis of research data.T test or chi square test were used for inter-group comparison,non-parametric rank sum test was performed for non-normal distribution data,Spearman correlation analysis was used for correlation analysis,and multivariate Logistic regression analysis was used to study the influencing factors of anxiety and(or)depression in patients with chronic heart failure.Results 1 Heart failure with anxiety and/or depression group and heart failure without anxiety and/or depression group of serum level of 5-HT contrast(327.43±55.69)vs(368.68±55.49),degree of culture for the junior middle school and the following: in 68 cases(70.1%)vs 13 cases(46.3%),heart failure course of more than 5 years: 36 cases(37.1%)vs 13 cases(19.4%)(P<0.05).2 AngⅡ levels in patients with heart failure complicated with anxiety and/or depression were compared according to cardiac function,NT-pro BNP and ejection fraction stratification respectively,Patients with grade cardiac Ⅳfunction(41 cases)compared with grade Ⅱ+ Ⅲpatients(56 cases)(25.26±3.70)vs(22.74±4.04)(P<0.05);Patients with NT-pro BNP>5180pg/ml(62 cases)compared with patients with NT-pro BNP≤5180pg/ml(35 cases)(24.43±4.29)vs(22.71±3.44)(P<0.05);Patients with LVEF<40%(33 cases)compared with patients with LVEF≥40%(64 cases)(25.05±4.25)vs(24.16±3.86)(P<0.05).3 Spearman correlation analysis was used to study 5-HT and AngⅡ,Ang(1-7)and blood lipids and other related indicators for all heart failure research subjects: 5-HT and AngⅡ,Ang(1-7),age,NT-pro BNP,LVM,TC,UA have no significant correlation(correlation coefficients are: 0.093,0.057,-0.002,0.102,-0.006,0.171,0.178).However,it has only a weak correlation with LDL-c(correlation coefficient 0.202,P<0.05).4 A Logistic multivariate analysis of all subjects with heart failure was carried out with anxiety and(or)depression as dependent variables.It was found that whether anxiety and(or)depression were associated with 5-HT,the course of heart failure was more than 5 years,and the educational level was junior high school and below(P<0.05),OR and 95%CI were 0.987(0.981~0.994),2.752(1.027~6.175)and 2.863(1.302~6.297),respectively.Conclusions 1 The higher the degree of heart failure in patients with chronic heart failure,the higher the serum AngⅡ level.2 There was no correlation between serum 5-HT and Ang Ⅱ,Ang(1-7)indexes in patients with chronic heart failure.3 5-HT level reduction,duration of heart failure over 5 years,and education to junior high school or below were influence factors for anxiety and(or)depression in patients with chronic heart failure.Figure 0;Table 11;Reference 119.
