| Objectives To investigate clinical nurses’ internal turnover tendency in a hospital through questionnaire survey and analyze the related factors,construct the model of fine performance nursing management based on the theory of fine management,observe it’s effect on nurses’ internal turnover tendency.Methods From October 2018 to November 2018,nurses working in a 3-A hospital in Tangshan City were selected to carry out a questionnaire survey using the revised Turnover Tendency Scale,the McCloskey/Mueller Satisfaction Scale and the Performance&Payment Satisfaction questionnaire,to analyze the current situation of nurses’ turnover tendency and the influencing factors.According to the intervention factors,a fine performance nursing management model was constructed based on the theory of fine management,which were put into practice from January 2019 to December 2019 to intervene ward nurses’turnover tendency.The main contents include:increasing the income of nurses in the ward,improving incentives on night shifts,increasing the weight of performance to the departments with high work intensity and high risk,adjusting policy of ward nurses transfer of posts,etc.After one year of intervention,nurses in clinical wards were evaluated again using the same questionnaires.SPSS17.0 statistical software was used for data analysis,measurement data were expressed as mean ± standard deviation(x±s),counting data were measured by chi-square test,with P<0.05 as the difference is statistically significant.Results 1 The present situation of ward nurses’ turnover tendency:total score is(10.83±3.25),turnover possibility dimension is(5.71±1.78),turnover motivation dimension is(5.12±1.69),ward nurses overall turnover tendency is high.2 Univariate analysis showed that professional title and length of working years are related to ward nurses turnover tendency,the difference is statistically significant(P<0.05);family support is related to ward nurses’ turnover tendency,the difference is statistically significant(P<0.001);department category,doing night shift,nurse-patient dispute experience,heavy workload,working overtime and satisfaction degree of head-nurse’s management,all these factors are strongly related to ward nurses’ turnover tendency,the difference is statistically significant(P<0.05);the McCloskey/Mueller Satisfaction score and the Performance&Payment Satisfaction are related to ward nurses’ turnover tendency,correlation coefficient is between-0.46 and-0.54.3 Multivariate analysis showed that the Performance&Payment Satisfaction level,doing night shift,the McCloskey/Mueller Satisfaction level,family support and working overtime are the influencing factors of ward nurses’ turnover tendency(P<0.05).4 Effect of the fine performance nursing management model showed that ward nurses turnover tendency decreased markedly after one year intervention(P<0.05),the average turnover rate decreased from 1.74%to 0.22%,the difference is statistically significant(P<0.05).Nurses’ McCloskey/Mueller Satisfaction level and Performance&Payment Satisfaction level improved significantly with statistical significance(P<0.05).Conclusions 1 Ward nurses’ turnover tendency score is(10.83±3.25),the overall turnover tendency is high.2 Multivariate analysis showed that the Performance&Payment Satisfaction level,doing night shift,the McCloskey/Mueller Satisfaction level,family support and working overtime are the influencing factors of ward nurses’turnover tendency(P<0.05).3 The fine performance nursing management model which based on fine management theory can reduce the turnover tendency effectively of nurses working in the clinical wards of the studied hospital.Figure 2;Table 18;Reference 188. |
