| Objectives To study the incidence of rotator cuff injury in patients with proximal humeral fracture and its correlation with age and fracture types. Methods From January 2015 to June 2018,98 patients with proximal humeral fracture who were operated in the Affiliated Hospital of Chengde Medical College were analyzed retrospectively.Among them, there were 28 males and 70 females;aged 23-91 years old, with an average of 62.6±16.3 years old;67 patients on the right side and 31 patients on the left side;82 patients underwent shoulder MRI before surgery, and 16 patients failed to undergo MRI based on MRI,24 patients were suspected of having rotator cuff injuries;8 patients with Neer 2 fractures,13 patients with 3 fractures, and 2 patients with 4 fractures;intraoperative exploration confirmed 23 patients with rotator cuff injuries and 75 patients without rotator cuff injuries.SPSS 19.0 statistical software was used to analyze the collected data. The measurement data(age)was expressed by (?)±s,and the comparison between groups was by t test;the count data(age group, gender, fracture type, etc.)was expressed by example(%).Chi-square test, continuous correction chi-square test or Fisher’s exact probability test were used for comparison.P <0.05 considered the difference to be statistically significant. The incidence of proximal humeral fracture with rotator cuff injury in different age groups was calculated and compared,The incidence of rotator cuff injury in different fracture types was calculated and compared according to neer fracture classification. Results Among 98 cases of fracture,23 cases(23.5%)of rotator cuff injury were found,15 cases(15.3%)of which were full-thickness tear. There was no significant difference in age and sex ratio between patients with and without rotator cuff injury(P > 0.05).There was a significant difference in the incidence of rotator cuff injury in patients of different ages(P=0.046).The highest incidence of rotator cuff injury was found in patients with rotator cuff injury ≥80 years old(6 / 10,60%),and the incidence of rotator cuff injury in women was higher than that in men. There was no significant difference in the fracture types between the patients with rotator cuff injury and those without rotator cuff injury(P=0.377),but the highest incidence of rotator cuff injury was found in Neer 3 patients(13 / 44,29.5%).Conclusions With the increase of age,the proportion of proximal humeral fracture with rotator cuff injury is gradually increasing;Neer 3 parts of proximal humeral fracture often with rotator cuff injury should be paid attention to;proximal humeral fracture often with rotator cuff injury should be repaired in time during operation to avoid related complications.Figure 15;Table 4;Reference 88... |
