ObjectivesIn this study,midnight-noon ebb-flow(Ziwu Liuzhu)moxibustion therapy was used to intervene in patients with postoperative fatigue syndrome(POFS)after hip arthroplasty(Hip Arthroplasty,HA),to explore their improvement of postoperative fatigue symptoms,psychological conditions and The effect of improving the quality of life after surgery and shortening the length of hospital stay lays the foundation for further research on the mechanism of timing moxibustion intervention in POFS,to verify the effect of time point selection on the efficacy of moxibustion,while highlighting the characteristics of traditional Chinese medicine nursing and promoting traditional Chinese medicine nursing development.MethodsA total of 86 patients with hip replacement surgery who were hospitalized in orthopedics in a tertiary Grade A hospital in Chengdu from October 2018 to December 2019 were selected,and 86 patients with POFS fatigue symptoms were randomized into an experimental group and a control group,43 cases in each one.The control group received routine moxibustion intervention,the treatment time was 8 a.m and 4 p.m;the experimental group provided patients with moxibustion intervention at10 am and 6 pm.The treatment time for both groups was from the first postoperative day to the seventh postoperative day,twice a day,each time for 20 minutes per hole.Sign the research informed consent and record the basic population sociological information 1 day before the operation,and the operation data will be collected and recorded on the day of the operation.Both groups of patients were evaluated at the same time period 1 day before surgery,1 day after surgery,3 days after surgery,and 7days after surgery,and the IFS(Identity Consequence Fatigue Scale,ICFS),Brief Profile of Mood States(BPOMS).Two weeks after the operation,the World Health Organization Quality of Life Scale-Brief Form Questionnaire(WHOQOL-BREF)was recorded.And record the total hospitalization time of the two groups of patients.ResultsA total of 75 patients completed the study,including 38 cases in the test group(4cases were discontinued and 1 case was excluded),and 37 cases in the control group(5 cases were discontinued and 1 case was excluded).The results show that:1.Evaluation of postoperative fatigueIn the experimental group,ICFS score were lower than the control group at the same time on the third day and the seventh day after intervention,and the difference was statistically significant(P<0.05).Repeated analysis of variance analysis of ICFS scores showed that ICFS scores were statistically significant in terms of intergroup effects,time effects,and interaction effects(P<0.05).The ICFS scores of the two groups of patients at each time point were further compared in pairs.2.Psychological evaluation effectThe BPOMS scores of the patients in the experimental group on the 3rd and 7th days after intervention were lower than those in the control group of the same period,the difference was statistically significant(P<0.05).Repeated measures analysis of variance for BPOMS scores showed that BPOMS scores were statistically significant in terms of group effects,time effects,and interaction effects(P<0.05).The BPOMS scores of the two groups of patients at each time point were further compared in pairs.3.Evaluation effect of the total time of hospitalizationThe total hospital stay in the test group was 8.65±1.92 days,and the control group was 10.89±2.21 days,the difference was statistically significant(P<0.05).4.Evaluation effect of quality of lifeThe two-week postoperative WHOQOL-BREF score was 79.89±4.55 in the test group and 73.38 ± 5.94 in the control group.The difference was statistically significant(P<0.05).ConclusionsUnder the guidance of the midnight-noon ebb-flow(Ziwu Liuzhu)theory,selective moxibustion can relieve fatigue symptoms and improve the postoperative psychological level of POFS patients in hip replacement surgery,shorten the totallength of hospitalization and improve the quality of life after surgery,Compared with conventional moxibustion therapy,its effect is better.Under the guidance of Ziwu Liuzhu theory,the timing of moxibustion can improve the efficacy of intervention in patients with POFS hip replacement surgery,promote the development of TCM time-effects and TCM nursing,it is worthy of clinical promotion and in-depth mechanism research.
