Objective:Under the guidance of the theory of the transfer function of spleen and stomach Qi,a comprehensive health management program of traditional Chinese medicine centering on type 2 diabetes mellitus patients is established,and a randomized,controlled clinical study is conducted.Through the overall evaluation of the effects of the model of TCM management that integrates the diet of syndrome differentiation and treatment,traditional exercise and five-element music on skeletal muscle of patients with type 2 diabetes mellitus,and it provides a new research approach for improving life management mode featuring TCM which is in line with the living habits of Chinese people,easy to operate and easy to promote.Methods:A randomized,controlled clinical study was conducted.From April 1,2018(solstice)to January 17,2020,type 2 diabetes patients are screened in the affiliated hospital of Chengdu university of traditional Chinese medicine,and a total number of107 subjects meeting the inclusion and exclusion criteria are selected.The included subjects are randomly divided into control group(n=52)and treatment group(n=55)by the principle of random number table.The control group is mainly based on diabetes education,outpatient doctor guidance and self-lifestyle management;the treatment group is under the guidance of a professional team,which is mainly based on the comprehensive management of the diet of TCM syndrome differentiation and treatment,Tai chi chuan and emotional nursing.The study period includes 12 weeks,with telephone follow-up at 2,6 and 10 weeks,and outpatient follow-up at 4,8 and 12 weeks,respectively.Hb A1c,pancreas islets function index and human body composition index are used as the main therapeutic indicators for statistical analysis.Results:1.baseline information:the baseline of the two groups was basically the same in terms of background data and clinical data(p>0.05),which was comparable.2. Evaluation results of the glucose metabolic indexes:Hb A1cwas significantly reduced in both groups after intervention compared with that at the beginning of enrollment(p<0.01).After 12 weeks,the improvement of the treatment group was better than that of the control group(p<0.05).3. Evaluation results of the islets function indexes:In both groups,HOMA-βincreased to a certain extent after intervention compared with that at the beginning of enrollment(p<0.05);For HOMA-IR there was a certain degree of decrease in thetreatment group compared with that at the beginning of enrollment(p<0.05),while there was no significant decrease in the control group compared with that at thebeginning of enrollment(p>0.05).For HOMA-IS and QUICKI,there was no significant change in the two groups compared with that at the beginning ofenrollment(p>0.05).After 12 weeks,there was no statistical difference in HOMA-β,HOMA-IR、HOMA-IS and QUICKI between the two groups(p>0.05).4. Evaluation results of human components related to skeletal muscle:Themuscle rate of the treatment groups increased after intervention compared with that at the beginning of enrollment(p<0.01),while there was no statistical difference in the control group(p>0.05);There was no statistical difference in the muscle storageability level(p>0.05);The protein content in the control group was increased to acertain extent compared with that at the beginning of the group(p<0.05),while there was no statistical difference in the treatment group.After 12 weeks,there wasstatistical differences in muscle rate between the two groups(p<0.05),while were no statistical differences in muscle storage ability grade and protein content(p>0.05).5. Evaluation results of TCM symptoms:The scores of symptoms of TCM in the two groups decreased significantly compared with that at the beginning of enrollment(p<0.01).After 12 weeks,the improvement value of TCM syndrome in the treatment group was significantly better than that in the control group(p<0.01).Conclusions:1.Patient-oriented model of TCM health management has a clear clinical efficacy in improving Hb A1cand muscle rate of type 2 diabetes patients without strict lifestyle strengthening,and the effect is better than the conventional guiding health management mode;it benefits the indictors such as HOMA-IR and HOMA-βto some extent,and it may plays a positive role in promoting the improvement of IR in patients.2.The TCM health management program guided by the theory of the transfer function of spleen and stomach Qi,proved to be effective and safe,has a clearcurative in improving the TCM symptoms of type 2 diabetes,which is worthy of application and promotion.
