Clinical Observation On"Erchen Tang And Sanzi Yangqin Decoction"in Treating Acute Exacerbation Of Chronic Obstructive Pulmonary Disease(Phlegm And Turbid Obstruction Of Lung Syndrome)

1.clinical research Objective: Based on the theory of correlation between sputum and airway mucus hypersecretion,by observing the clinical efficacy of Erchen tang combined with Sanzi Yangqin decoction in the treating AECOPD(syndrome of turbid phlegm obstructing the lung),and to evaluate the efficacy and safety of Erchen tang combined with Sanzi Yangqin decoction in treating AECOPD mucus hypersecretion.Method: From the inpatient department of respiratory department of the affiliated hospital of Chengdu University of TCM,60 patients with AECOPD(syndrome of turbid phlegm obstructing the lung)were randomly divided into treatment group(n = 30)and control group(n = 30).The treatment group was treated with Erchen tang and Sanzi Yangfu decoction combined with canonical western medicine,at the same time the control group was treated with only canonical western medicine for 2 weeks.During the observation period,the therapeutic efficacy and safety indexes of the two groups were collected and analyzed by statistical software SPSS21.0.Results:1.In terms of the curative effect of TCM syndrome,both the treatment group and the control group achieved certain curative effects(P < 0.05),which was statistically significant,and the treatment group was better than the control group(P < 0.05).2.In terms of single TCM syndrome score,CAT score and m MRC score,the single TCM syndrome score,CAT score and m MRC score of the two groups after treatment were significantly lower than those before treatment(P < 0.01).After treatment,the scores of CAT,m MRC,cough,sputum,shortness of breath and abdominal distension in the treatment group were better than those in the control group(P < 0.05).And wheezing,chest tiredness and fatigue showed no statistical significance between the two groups(P > 0.05).3.In terms of pulmonary function and blood gas analysis,pulmonary function FEV1,FEV1/FVC,FEV1%pred,MVV and arterial blood gas Pa CO2,Pa O2,Sa O2 were significantly improved in both groups after treatment.After treatment,the improvement of pulmonary function FEV1,FEV1/FVC,MVV and arterial blood Pa O2 and Sa O2 in the treatment group was better than that in the control group(P < 0.05).FEV1%pred and Pa CO2 showed no significant difference between the two groups after treatment(P > 0.05).4.The values of peripheral blood WBC,NE and serum TNF-α in the two groups were significantly improved after treatment as compared with those before treatment.The improvement of NE and serum TNF-α in the treatment group was better than that in the control group(P < 0 05).Statistical result was no significant difference in white blood cell count between the two groups after treatment(P > 0.05).5.During the period of clinical observation,the safety indexes of the two groups were not significantly abnormal,there were no adverse drug reactions during the study.Safety evaluation is grade 1.Conclusion: Erchen tang and Sanzi Yangqin decoction combined with routine treatment of western medicine can effectively control clinical symptoms,improve quality of life,lung function and inflammation index of AECOPD(syndrome of turbid phlegm obstructing lung)than routine treatment of western medicine alone,which provides a new idea for the combination of Chinese and western medicine in the treatment of AECOPD(syndrome of turbid phlegm obstructing lung).2.It enriches the scientific connotation of "visible phlegm".Through the correlation between visible phlegm and airway mucus hypersecretion,Erchen decoction and Sanzi Yangqin decoction are used to guide clinical medication to achieve a good curative effect.this mechanism may be related to the fact that Erchen decoction combined with Sanzi Yangjin decoction can effectively inhibit airway mucus hypersecretion in patients...
