Clinical Study On Repair And Functional Reconstruction Of Soft Tissue Defect Under Submentel Island Flap Immediately After Oral Carcinoma

Objective:To study the immediate repair and functional reconstruction of submentel island flap for soft tissue defects after oral cancer.Method:The retrospective approach method was used to select 61 patients who received free forearm flaps,repair and functional reconstruction after resection of oral carcinoma from April 2010 to August 2016 at the affiliated Huaian No.1 People's Hospital of Nanjing Medical University.27 cases of repair and functional reconstruction of free forearm flap were taken as group A,and 34 cases of repair and functional reconstruction were taken as group B.SPSS23.0 statistical software was used to compare and analyze the clinical data of the two groups,the average operation time,the amount of exudate after surgery,the survival of the flap,the infection or necrosis of the flap,and the satisfaction of the patients.The two groups of patients were followed up.During the follow-up,the functional recovery and appearance of the two groups of patients were observed and recorded,and the quality of life after operation was evaluated.Results:In group B,which was repaired and functional reconstructed with submentel island flap,the average operation time and fluid exudation were significantly lower than in group A,which was repaired and functional reconstructed with free forearm flap,andthe comparison was statistically significant(P< 0.05).The survival rate of flap in the two groups was not statistically significant(P > 0.05),and the necrosis rate of flap infection in the B group was significantly lower than that of the A group,and the comparison was statistically significant(P < 0.05).There was no statistical significance in the two groups of functional recovery(P > 0.05),the functional recovery of the area after surgery in group B was better than that in group A(P <0.05),and there was no statistical significance in the comparison of UW-QOL scores in the two groups(P > 0.05).The total satisfaction of flap after operation in group B was significantly higher than that of group A,and the comparison was statistically significant(P< 0.05)Conclusion:The survival rate of the soft tissue defect after oral cancer resection was similar,but the function of the feeding area was restored well after the repair of the submentel island flap and the free forearm flap,and the satisfaction of the patients was also improved.Therefore,clinically speaking,under the strict control of indications,the submentel island flap has certain advantages in the clinical evaluation during and after surgery.
