Objective:To investigate the safety and efficacy of interventional therapy for balloon predilatation with different diameters in severe carotid artery stenosis.Methods:100 patients with symptomatic severe carotid artery stenosis admitted to our hospital from January to September 2018 were selected.They were randomly divided into 4mm balloon predilection group and 5mm balloon predilection group,with 50 cases in each group.The operation success rate,residual stenosis rate,changes of heart rate and blood pressure and complications were compared between the two groups.Results:The two groups of baseline data were comparable.The success rate of both groups was 100%.The average preoperative vascular stenosis in the 4mm balloon predilection group was(84.8±8.6)%,and postoperative residual stenosis was(12.2±6.2)%.In the 5mm balloon predilatation group,the average preoperative vascular stenosis was(82.1±8.0)%,and the postoperative residual stenosis was(6.4±6.1)%.There was no difference in preoperative vascular stenosis between the two groups,and the postoperative residual stenosis rate was significantly lower than that before the operation,and the residual stenosis rate of the 5mm balloon predilation group was lower than that of the 4mm balloon predilation group(P<0.05).In the 4mm balloon preexpansion group,systolic blood pressure decreased(13.0±33.2)mm Hg and heart rate decreased(11.0±9.7)times/min.The systolic blood pressure decreased(14.0±28.5)mm Hg and the heart rate decreased(7.0±10.8)times/min in the 5mm balloon preexpansion group.There was no significant difference in blood pressure and heart rate between the two groups(P>0.05).There were 28 cases of complications(carotid sinus reaction,hyperperfusion syndrome,in-stent restenosis,stroke,arterial dissection,and external carotid artery involvement)in the 4mm balloon predilection group,and 25 cases in the 5mm balloon predilection group,with no statistical difference.Conclusion:The improvement of balloon predilatation in 5mm is better than that in 4mm.Both groups were safe and effective in interventional treatment of severe carotid artery stenosis.
