| Objective: The aim of the study was to assess the influence of car otid endarterectomy(CEA)on cerebral perfusion change and cognitive pe rformance in patients with high grade carotid artery stenosis?Methods:A total of 30 patients with unilateral severe carotid stenosis(?60%)were involved in the study.CTP examinations were performed about 1week before and after carotid endarterectomy to collect perfusion para meters.Montreal Cognitive Assessment scale(Mo CA)and Mini-Mental S tate Examination(MMSE)scores were collected 1 week before and 3-4 months after CEA?Results: 1.After CEA,a significant improvement was observed in Mo CA,sub-tests of “abstraction,” “attention,” and “recall”in Mo CA sho wed better improvement than other sub-tests(P<0.05);But there was no significant change in MMSE scores after CEA;2.After CEA,significant improvement was observed in cerebral perfusion in the ipsilateral side,mainly in CBV(3.96 ± 1.52 vs 5.42 ± 1.90),CBF(50.11 ± 18.27 vs 64.22 ± 23.05),MTT(6.08 ± 1.33 vs 4.70 ± 1.10),r CBV(0.85 ± 0.29vs 1.45 ± 0.72),r CBF(0.82 ± 0.26 vs 1.13 ± 0.24),and the differenc es were statistically significant(P<0.05);3.Spearman's correlation analys is between MTT and Mo CA scores demonstrated a linear relationship w ith an excellent correlation coefficient(rs=-0.907,P<0.001);4.After multiple linear regression analysis,there was a positive correlation betwe en Mo CA and educational level,and the difference was statistically sign ificant.There was no significant correlation between cognitive improvement and general characteristics(age,gender,hypertension,diabetes,hyper lipidemia,coronary heart disease,smoking,surgical side,education leve l)?Conclusions:For patients with high grade carotid stenosis,carotid endarterectomy can not only reduce the clinical symptoms and improve cerebral perfus ion,but also improve cognitive function by improving cerebral perfusion.Thus,brain reperfusion was an important factor for cognitive improve ment?... |
