The Construction And Application Of The Sensitive Index System Of Nursing Quality In Interventional Surgery Based On Three-dimensional Quality Structure Theory

Objective This study based on the theory of "three dimensional quality structure",build a set of scientific,quantitative and practical step in the operating room nursing quality sensitive index system,to regulate in the content and elements of the operating room nursing quality evaluation,make intervention in the operating room nursing service standardization and standardization,promote the continuous improvement in the operating room nursing quality.Methods Using literature review method and semi-structured interview method,based on the theory of "three-dimensional quality structure",the system of nursing quality sensitivity index for interventional operating room was preliminarily formulated.Delphi expert consultation method was adopted to carry out two rounds of expert consultation to screen and determine the sensitive index of nursing quality in interventional operating room,and the reliability and representativeness of expert consultation results were evaluated by expert positive coefficient,expert authority coefficient,Kendall coordination coefficient and concentration degree of expert opinion.According to the results of the second round of expert consultation,the analytic hierarchy process(AHP)was adopted to establish the hierarchical structure model,construct the judgment matrix of pairwise comparison,determine the weight of each level of indicators,and conduct consistency test.The sensitive index of nursing quality in the interventional operating room was applied to the clinic by empirical research to further verify the scientificity and practicability of the index system.The interventional operating room of a grade a hospital in Inner Mongolia was selected as the research object.September 1,2019--September 31,2019 is a monitoring point before the implementation of the sensitive index system of nursing quality in interventional operating room,and the conventional nursing quality management mode in interventional operating room is implemented in accordance with the relevant system of hospital quality control.From October 1,2019 to October 31,2019 is a monitoring point after the implementation of the nursing quality sensitive index system in the interventional operating room.By comparing the incidence of adverse events before and after the implementation of the interventional operating room nursing quality sensitive index system and the satisfaction of patients and doctors to the nursing work in the interventional operating room,the improvement effect of the quality of interventional operating room nursing was evaluated.Excel 2003,SPSS 22.0and yaahp10.1 were used for data processing.Results In this study,the system of nursing quality sensitivity in interventional operating room was constructed,including 3 first-level indicators,9 second-level indicators and 33third-level indicators.In this study,21 qualified experts were finally included.After two rounds of expert consultation,the effective data were analyzed,and the results showed that the positive coefficient of both rounds of experts was 100%.The authority coefficients of the two rounds of experts were 0.85 and 0.86 respectively,and the Cr values of the two rounds of experts were >0.7,indicating that the expert consultation results were reliable.The Kendall coordination coefficients of the two rounds were 0.135 and 0.169 respectively,and the chi-square test P<0.05.The results of AHP are as follows: the weight coefficient of primary index is 0.1335,the weight coefficient of secondary index is 0.6551,and the weight coefficient of tertiary index is 0.2114.The empirical results showed that after the implementation of the sensitive index system of nursing quality in interventional surgery,the incidence of adverse events in interventional surgery was lower than that in the control group(P<0.05),and the satisfaction of interventional surgery patients and doctors to interventional surgery was higher than that in the control group(P<0.05).Conclusion Based on the theory of "three-dimensional quality structure",through literature review and semi-structured interview,and through two rounds of expert consultation,this study finally constructed a system of sensitive indicators of nursing quality in interventional operating room,including 3 first-level indicators,9 second-level indicators and33 third-level indicators.According to the results of the second round of expert consultation,the analytic hierarchy process(AHP)method was used to establish the hierarchical structure model,construct the judgment matrix of panttwo comparison,determine the weight of indicators at all levels,and conduct the consistency test.The CI value of all indicators was less than 0.1.The judgment matrix constructed in this study had good consistency,and the weight results of indicators at all levels were acceptable.The empirical study further verified the scientificity and practicability of the sensitive index system of nursing quality in interventional operating room.This study has achieved the desired effect,established a relatively complete set of interventional operating room nursing quality sensitive index system,for clinical nursing management to provide a set of strong operability,good practicality evaluation tools,highlighting the characteristics of interventional operating room work,pay attention to specialty characteristics,improve the quality of interventional operating room nursing.
