| Objectives To explore the influencing factors of memory dysfunction in stroke and hypertension patients,so that can provide a theoretical basis for formulating early nursing measures and redue the incidence of memory dysfunction. Methods The study was a case-control study. The stroke and hypertension patients were selected in October 2018 to September 2019 in north China university of science and technology affiliated hospital neurology hospital treatment, and in patients admitted to hospital after a week,in patients with no other discomfort body, does not affect the disease treatment, using assessment tools Rivermead behavior memory test the second edition(RBMT-II)selected case group and the control group. The selected 202 patients with stroke and hypertensive comorbidities with memory function score ≤21 points were selected as the case group, and the patients with stroke and hypertensive comorbidities with memory function score ≥22 points were selected as the control group. The influence factors of two groups of patients, respectively, using questionnaire including general demographic factors, disease treatment factors, mental factors, social support factors, sleep, before the disease of lifestyle factors, environmental factors were investigated and comparative analysis of patients in stroke and hypertension in the basic information, disease and treatment, clinical symptoms, the psychological conditions, the distribution of social and family support status differences.Data obtained from the survey were input and sorted out by Excel software, and then statistical analysis was carried out by SPSS17.0 and Graph Pad Prism8.0 software.Descriptive statistical analysis was used for general demographic factor,Chi-square test was used for single factor analysis.Multivariate Logistic regression analysis was performed. Results 1 Single factor analysis of memory dysfunction in stroke and hypertension patients The results showed that, gender, age, cultural degree, personality type, family income, work status, professional type, nature of work, living place, the sleep quality, sleep need drugs, depression, feeling down, often blame myself, feel lonely, was not interested in things, feel recovery after treatment, fear of disease prognosis, social support, trust in with doctors, family care, exercise frequency, daily communication, reading or learning situation, word adverse, limb hemiplegia, incontinence, dizziness, headache, daily life activities ability, the number of course of stroke, incidence of stroke, staging and risk of hypertension, oral antihypertensive drugs on time, and whether there were multiple focal sites of stroke were related to the occurrence of memory dysfunction in patients with stroke and hypertension, and the difference was significant(P<0.05).2 Multivariate Logistic regression of memory dysfunction in stroke and hypertension patients analysis showed that, age in 60~74(OR=2.418),age in 75~84(OR=2.748),without reading or studying(OR=2.571),limb hemiplegia(OR=2.178),often incontinence(OR=5.888),sometimes feel dizzy(OR=2.747),often dizzy(OR=3.016),high blood pressure in stages and risk stratification for heavy(OR=2.686),for multiple stroke lesion(OR=1.679),the sleep quality was poor(OR=2.395)are risk factor for memory dysfunction in stroke and hypertension patients.College degree or above(OR=0.223)and medium-high level of social support(OR=0.388)are protective factors for memory dysfunction in stroke and hypertension patients(P<0.05).Conclusions Age,reading or learing, hemiplegia, incontinence, dizzness, hypertension stage and risk stratification, lesion site of stroke, sleep quality, educational level, and social support are the influencing factors for memory dysfunction in stroke and hypertension patients. The older the patients with stroke and hypertension patients, the higher the frequency of incontinence and dizziness, the heavier the hypertension stage and risk stratification, the more severe the hemiplegia, the more lesion sites of stroke, the worse the sleep quality, the greater the risk of memory dysfunction. The lower the frequency of reading or learning, the greater the risk of memory dysfunction in stroke and hypertension patients. The higher the educational level and the higher the social support level, the lower the risk of memory dysfunction instroke and hypertension patients.Figure1;Table13;Reference 135... |
