Objective To study the biomarkers of lipoproteins associated phospholipase A2(Lp-PLA2),high sensitive C-reaction protein(hs-CRP)and N-terminal pro B type natural peptide(NT-proBNP),The relationship between the biomarkers and the vulnerability of coronary artery plaque and the predictive diagnostic value of the biomarkers are expected to provide new clinical evidence for screening high-risk population of coronary heart disease(CHD)in clinical work.Methods 1 The subjects of this study were 139 patients who met the inclusion criteria and had been undergo coronary angiography(CAG)from January 2018 to June 2019.According to CAG results,139 patients were divided into coronary heart disease group(104 cases)and control group(35 cases).2 According to the results of VH-IVUS,104 patients were divided into two groups: unstable plaque group(57 cases)and stable plaque group(47 cases).3 All the selected patients were tested for biological indicators such as Lp-PLA2,hs-CRP and NT-proBNP.4 To analyze the relationship between the the biomarkers Lp-PLA2,hs-CRP and NT-proBNP and vulnerable plaque.Results 1 There was no significant difference in age,gender,BMI index,the proportion of hypertension and diabetes,the history of smoking and drinking among the three groups(P > 0.05).2 The general biochemical indexes of unstable plaque group,stable plaque group and control group were compared.TC,TG,LDL-C and other general biochemical indexes of unstable plaque group were higher than those of stable plaque group and control group,the difference was not statistically significant(P > 0.05).3 The biological markers of Lp-PLA2,hs-CRP and NT-proBNP in the three groups were significantly higher than those in the control group,and the difference was statistically significant(P < 0.01).4 The plaque area,plaque load,lumen area and elastic membrane area in unstable plaque group were higher than those in stable plaque group,The percentage of vascular remodeling index,necrotic core area and fibrofatty tissue area in unstable plaque group was higher than that in stable plaque group,while the percentage of calcified tissue area and fibrofatty tissue area in unstable plaque group was lower than that in stable plaque group(P < 0.05).5 The results of logistic analysis showed that the or value of Lp-PLA2 was 1.026,that of hs-CRP was 2.083 and that of NT-proBNP was 1.006,which indicated that Lp-PLA2,hs-CRP and NT-proBNP were the risk factors of unstable plaque of coronary heart disease(P < 0.01).6 The area under ROC curve showed that the area under Lp-PLA2,hs CRP and NT-proBNP curve was 0.727,0.734 and 0.717,respectively,which were greater than 0.5.When the area under ROC curve was 0.7-0.9,it indicated that it had certain accuracy,that is,Lp-PLA2,hs-CRP and NT-proBNP had predictive diagnostic value for vulnerable plaque.Conclusions 1 hs-CRP,Lp-PLA2 and NT-proBNP in patients with coronary heart disease are significantly increased,which indicates that hs-CRP,Lp-PLA2 and NT-proBNP have obvious correlation with coronary heart disease.2 Lp-PLA2,hs-CRP and NT-proBNP are correlated with the instability of vulnerable coronary plaque.3 Lp-PLA2,hs-CRP and NT-proBNP are risk factors for unstable plaque of coronary heart disease,and have certain value in predicting and diagnosing vulnerable plaque.Figure 1;Table 5;Reference 135...
