Objective:The value of applying the 2012 Atlanta Classification of Acute Pancreatitis and new Japanese severity score?New JSS?in assessment of severity of clinical acute pancreatitis.Methods:Selected the patients who were admitted to the Affiliated Hospital of Hangzhou Normal University from September 2017 to September 2019 with acute pancreatitis and used the 2012 Atlanta Classification of Acute Pancreatitis and JPN guidelines for the management of acute pancreatitis?2015?about the severity of clinical acute pancreatitis within 72 hours after admission respectively to record the examination within 72 hours after the onset of the disease,the result,the changes of life signs,AP patient's condition after recovery,length of stay and so on in detail.Finally,the original data were sorted out and the statistical significance of the difference was tested by a reasonable statistical method.Results:The total age distribution of 86 patients was 50.18±17.55 years old?18-88years old?,including 51 males?59.3%?,average age 47.94±17.13 years old,35females?40.7%?,average age 53.46±17.88 years old;Mean duration of onset:16.20±17.91 hours;Average length of stay:13.70±7.09 days;In terms of pathogeny:47 cases of biliary origin?54.7%?,29 cases of hyperlipidemia?33.7%?,10 other cases?11.6%?;27 cases of smoking habits?31.4%?,24 cases of drinking habits?27.9%?;19 cases of hypertension?22.1%?,9 cases of diabetes mellitus?10.5%?;6 cases of death?7.0%?,2cases of men?33.3%?,4 cases of women?66.7%?,There were 4 cases of biliary origin?66.7%?,1 case of hyperlipidemia?16.6%?,and 1 case of other etiology?16.6%?.In the 2012 Atlanta Classification of Acute Pancreatitis,MAP,MSAP and SAP had significant difference in death rate of different severity and there was statistical significance between the length of stay in hospital?P<0.05?.In terms of test indexes,WBC,SCRP,PLT,Ca2+,BUN and Crea had statistical significance?P<0.05?,LDH had statistical significance?P<0.01?;Under the new Japanese severity score?New JSS?,AP severity and smoking habit had statistical significance?P<0.05?,and length of stay had statistical significance?P<0.01?;In terms of test indexes,WBC and Alb had statistical significance There were significant differences in SCRP,HCT,PLT,LDH,Ca2+,BUN and Crea?P<0.01?.Based on the 2012 Atlanta Classification of Acute Pancreatitis,the new JSS scoring system has good prediction for organ failure(AUCROC=0.774,sensitivity 61.5%,specificity 93.3%).And under the 2012Atlanta Classification of Acute Pancreatitis,SAP(AUCROC=0.963,sensitivity 92.9%,specificity 90.3%)and death(AUCROC=0.733,sensitivity 66.7%,specificity 80.0%).Conclusion:Based on the 2012 Atlanta Classification of Acute Pancreatitis,new JSS scoring system has a certain practical value for disease assessment.And it is effective,relatively simple and has a high clinical application value.
