| Objetive: To evaluate the preventive effect of injecting Chinese medicine Qingyi Decoction through endoscope in endoscopic retrograde cholangiopancreatoraphy(ERCP)on post-ERCP pancreatitis(PEP)and post-ERCP hyperamylasemia(PEH)and analyze the risk factors of post-ERCP pancreatitis and hyperamylaseemia.Methods: This study was selected from the inpatients were diagnosed of choledocholithiasis at the Second Hospital of Hebei Medical University from February 2019 to December 2019 were planned to undergo ERCP.After designing experiments,they were randomly divided into groups to collect data.The experimental group was injected with 100 ml Qingyi Decoction through endoscope,and the control group was injected with 100 ml saline.The incidence of PEP and PEH was compared between the two groups,and the preventive effect of Qingyi Decoction on PEP and PEH was finally determined,and determine the relevant risk factors for PEP and PEH by logistic regression analysis.Results: Among the 219 patients included,PEP occurred in a total of 17 cases,the incidence rate was 7.76%(17 cases / 219 cases);43 cases were diagnosed as PEH,the incidence rate was 19.63%(43 cases / 219 cases).The incidence of PEP and PEH in the Chinese medicine group was significantly lower than that in the control group(P<0.05),and the effect of reducing the serum amylase in the Chinese medicine group was better than that in the control group(P<0.05).According to the analysis of risk factors,periampullary diverticula and the basket accesses the pancreatic duct were independent risk factors of PEP.Gender,the diameter of bile duct≥1.0cm,operation time ≥35 minutes,the basket accesses the pancreatic duct were independent risk factors of PEH,pancreatic stent is an independent protective factor of PEH.Conclution: The preventive effect of injecting Chinese medicine Qingyi Decoction through endoscope on PEP and PEH is obvious,and it is a protective factor for PEP and PEH.Periampullary diverticula and the basket accesses the pancreatic duct were independent risk factors for PEP.Gender,the diameter of bile duct≥1.0cm,operation time ≥35 minutes,the basket accesses the pancreatic duct were independent risk factors for PEH,pancreatic stent is an independent protective factor for PEH. |
