Objective: To evaluate the prognostic value of preoperative peripheral blood neutrophil to lymphocyte ratio in patients with pancreatic ductal adenocarcinoma after pancreaticoduodenectomy.Methods:The relevant data of patients who underwent pancreaticoduodenectomy in the Second Hospital of Hebei Medical University from January 1,2016 to May 30,2019 were collected retrospectively.Receiver operating characteristic(ROC)curve analysis method was used to draw the ROC curve of patients’ NLR.When the Jordan Index(sensitivity + specificity-1)reached the maximum,the corresponding NLR value was taken as the best cut-off value.The patients were divided into two groups: high NLR group and low NLR group.The survival analysis was performed by Kaplan-Meier curve the difference between groups was analyzed by Log-Rank test and multi-factor analysis was conducted by Cox curve.Results: According to the ROC curve,2.69 is the cut-off value.Univariate analysis of NLR,age,PLR,carbohydrate antigen 199 and intravascular tumor thrombus,T stage,comprehensive stage,and degree of differentiation were indicators that affect the prognosis of patients(P <0.05);The 1-year s-urvival rates of high NLR group and low NLR group were 51.61% and 43.33%,and the 2-year survival rates were 9.68%and 13.33%.Cox multivariate analysis NLR,PLR,CA-199 and intravascular tumor thrombus were independent risk factors that affect the prognosis of patients..Conclusion:Preoperative high NLR in patients with pancreatic cancer undergoin-g pancreaticoduodenectomy is an independent risk factor for poor prognosis and can be used as an independent indicator to evaluate the prognosis of patients with pancreatic cancer. |
