| Objective By comparing and analyzing the clinical indicators of the axillary pure space approach and modified traditional Judet approach for the treatment of scapular neck and glenoid fractures,the safety and clinical effect of the axillary pure gap approach in the treatment of scapular neck and glenoid fractures were discussed.Methods Choose between January 2005 and December 2018 in chifeng city hospital diagnosis of neck,shoulder blade,after fracture of the 42 patients as the research object,according to different grouping operation method,into the road side of axillary pure clearance for the experimental group(group A),24 cases,traditional modified Judet approach for the control group 18 cases(group B),to evaluate the effect of two groups of operation and analysis.Prior to surgery,all patients must undergo imaging tests to confirm the diagnosis.Patients were screened according to the inclusion criteria,and preoperative examination of relevant blood biochemistry and coagulation was completed.Surgical treatment was performed on the basis of no combination of other severe diseases.Patients in the two groups were assigned special personnel to guide functional exercise until fracture healing after surgery.Intraoperative record relevant data of two groups of patients,including patients with fracture of shoulder neck,dishes in surgery unavailable(min),incision length(mm),intraoperative blood loss(ml),flow rate(ml)and postoperative evaluation(Hardegger),application of shoulder joint function after surgery for 6 months after limb with shoulder joint evaluation criteria(Constant-Murley)scores,and statistical analysis,evaluation of different surgical methods in the treatment of shoulder neck,jar fractures near,medium-term curative effect.Results1.Compared with each other,the total length of surgical incision,intraoperative blood loss,operation time,drainage volume,and postoperative assessment of shoulder function(Hardegger)were statistically significant(P <0.05).2.Postoperative Shoulder Joint Evaluation Standard(Constant Murley): Activities of daily living ADL,Manual muscle test(MMT),and Range of motion(ROM)were statistically significant(P <0.05),and there was no significant difference in visual analogue scale(VAS)(P>0.05).Conclusion1.The pure clearance approach at the axillary margin,with reasonable incision design,full exposure of the operative field,maximized muscle integrity,significantly reduced intraoperative blood loss and drainage,can avoid major blood vessels and important nerve damage,so the operation is smooth,postoperative recovery can be rapid.2.The axillary margin pure space approach is a relatively good surgical treatment method for scapular-neck and glenoid fractures,which is worthy of clinical application.3. |
