Study On The Metabolism Of Nondepolarizing Muscle Relaxant Vecuronium Bromide In Patients With Extensive Burns

Abstract/Summary:
Objective Each year,approximately 11 million people worldwide[1]have different degrees of burns and scalds.The diagnosis and treatment of patients after burns has become the most important thing.In this study,we compared the application of the non-depolarized muscle relaxant vecuronium in orthopedics and burn patients to study the metabolism of the non-depolarized muscle relaxant vecuronium in patients with extensive burn.Methods Select 50 burn patients admitted to hospital during the period from December 2017 to December 2019 and 50 orthopedic patients with general anesthesia during the same period as the observation group and control group.Observe the heart rate,Blood pressure,as well as statistics of the amount of vecuronium used as a muscle relaxant and the duration of blockade and blood samples collected at the prescribed time,were used to detect the concentration of vecuronium and the expression of n ACh R.The data were sorted out.Statistical analysis was performed using SPSS 25.0software.Counting data were expressed as frequency and percentage.Comparison between groups was performed using Pearson/fisher chi-square test or Fisher exact probability method.Measurement data that conformed to the normal distribution were described by mean±standard deviation.The independent sample t test was used for comparison between groups,the median and quartiles were used for non-normal distribution data,and the rank sum test was used for comparison between groups.Results The amount of muscle relaxant in the burn group was significantly greater than that in the patients with bone injury(P<0.05),and the maintenance time of non-depolarized muscle relaxant in the burn patients was significantly shortened(P<0.05).The blood pressure and heart rate of the burn patients group were significantly higher than those of the orthopedic patients group(P<0.05).There was no difference in blood concentration after5 minutes of administration(P>0.05).After 30 minutes,the blood concentration of muscle relaxant in the burn group was lower than that in the bone injury group(P<0.05).The expression ofεn-ACh R was significantly lower than that of the bone injury group(P<0.05),and the expression ofγn-ACh R was higher than that of the bone injury group,the difference was statistically significant(P<0.05).Conclusions After the patient is burned,the body is in a high-dynamic,high-metabolism state,coupled with an increase in the expression ofγn-ACh R in the patient’s body,which causes the body to have low-sensitivity resistance to NDMR,which ultimately leads to the non-depolarized muscle relaxant drug dose during surgery Significantly increased and drug metabolism accelerated.
