| Coronary angiography is the golden standard for diagnosis of coronary artery disease,judging the severity of the stenosis and evaluating the indication of percutaneous coronary intervention.It can anatomically evaluate the coronary artery,but fails to elucidate the association between the degree of stenosis and myocardial ischemia in a functional way.At present,the strategy of revascularization is gradually changing from anatomy to function.Fractional flow reverse,a commonly used invasive functional index to assessing coronary artery disease,can provide objective basis for percutaneous coronary intervention by matching the relationship between stenosis and blood flow in an objective and accurate way.However,the use of vasodilators during the testing process limits the clinical application of fractional flow reverse.Instantaneous wave-free ratio can make a functional evaluation of coronary artery lesions without using vasodilators,and has good consistency and similar diagnostic accuracy as same as fractional flow reserve.Therefore,the basic principle,definition,measuring methods and guiding effect of instantaneous wave-free ratio in revascularizationare discussed. |
