OBJECTIVE:To measure the preoperative and postoperative laryngeal anatomical structures of patients with glottic laryngocarcinoma by scanning with CT and analysis the relevance of the collected data.Then to explore the effect of morphological parameters of the larynx on postoperative laryngeal function.The aim of the study is to find a new method for evaluating laryngeal function after laryngeal cancer surgery,thereby providing assistance for the clinical treatment of laryngeal cancer.METHODS : We selected 86 patients with glottic laryngeal cancer who were diagnosed by pathology and had undergone vertical frontolateral partial laryngectomy.We used CT to measure the preoperative and postoperative values of the larynx,then we followed up and recorded their situation of decannulation,total score of voice handicap index(VHI),etc.The collated data were statistically analyzed using statistical software named SPSS 21.0.RESULTS: The changes of the ratio of preoperative and postoperative cross-sectional area of larynx had the greatest impact on various indicators for assessing laryngeal function,and was negatively correlated with the total score of VHI,and positively correlated with MPT and the rate of decannulation.The changes of the ratio predicted the overall accuracy of whether extubation can be performed after surgery was 88.4% in the logistic regression model,and its accuracy was more advantageous than the absolute value of postoperative cross-sectional area of larynx.It believed that there was a high chance to decannulate and get a good voice quality when the ratio of preoperative and postoperative cross-sectional area of larynx was above 30.90% and the bilateral thyroid cartilage both retained more than 3/5.Conclusion: The relative values of the parameters of various anatomical structures of the larynx by measuring with CT were more suitable than the absolute values to evaluate the changes oflaryngeal morphology and function.And to some extent,the relative values “cross-sectional area of larynx” can estimate the recovery of postoperative laryngeal function,especially the decannulation.This can be used as a new reference for clinical treatment of laryngeal cancer.
