| Objective: Our study was to observe the effect of iontophoresis of the iontophoresis of Huoxue Sanyu tincture on on prethrombotic state in patients with lumbar intervertebral disc herniation after operation to investigate the feasibility of using iontophoresis of traditional Chinese medicine to prevent thrombosis formation in deep veins of lower limbs after spinal operation.Method: From February 2019 to January 2020,60 patients who were treated by operation about lubar intervertebral disc protrusion were selected from the department of orthopaedics of the Second People’s Hospital affiliated to Fujian University of Traditional Chinese Medicine.The patients were randomly divided into treatment group and control group and each group including 30 patients.All patients underwent single-level lumbar discectomy combined with interbody fusion and internal fixation under general anesthesia.The treatment group was given Huoxue Sanyu tincture iontophoresis intervention on the 1st day after operation and the control group was given normal saline iontophoresis,the frequency of which was 20 minutes / time,twice a day and lasted to the 7th day after the operation.Blood samples were taken on 1st day before the operation,and on 1 day,3er days and 7th days after operation to observe the changes of PT,APTT,TT,FIB,D-dimer and thrombelastogram to analyze the state of coagulation function.Color Doppler ultrasonography was performed on the 1st day before operation and on the 7th day after operation to estimate whether DVT occurred.Results:(1)There was no significant difference in the level of indexes about coagulation between the two groups on the 1t day before operation and on the 1st day after operation,which had comparability(P>0.05).The values of PT,APTT,TT,R,K decreased and FIB,D-dimer,α angle and the values of MA and CI increased of the patients in the two groups on the 1st day after operation compared with the date on the 1st day before operation,which was statistically significant(P<0.05).(2)PT,APTT and TT showed trend with gradual prolongation and FIB and D-dimer showed trend with decrease on the 3th and 7th day after operation in the patients of two groups,which was statistically significant(P<0.05).The comparison of APTT,TT,FIB,D-dimer in the two groups was statistically significant(P<0.05),but that of PT was not statistically significant(P>0.05).(3)The value of R and K showed trend with gradual increase and the value of α angle and MA showed trend with decrease on the 3th and 7th day after operation in the patients of two groups,which was statistically significant(P<0.05).The comparison of the value of R and MA and α angle in the two groups was statistically significant(P<0.05),but that of the value of K was not statistically significant(P>0.05).The tendency of hypercoagulability was similar between the two groups after operation,but the CI value of the treatment group was lower than that of the control group on the 3rd and 7th day after operation in trems of the coagulation composite index(CI),which was statistically significant(P<0.05).(4)No DVT occurred after operation in both groups.Conclusion: It is a safe and effective method to prevent the formation of DVT after spinal operation that the iontophoresis of Huoxue Sanyu tincture can reduce the level of molecular markers of prethrombotic state and improve the hypercoagulable state of blood. |
