Objective: To evaluate the value of facial nerve reconstruction by diffusion tensor imaging(DTI)in preserving facial nerve function after the surgery of acoustic neuroma. Methods: A total of 62 patients with acoustic neuroma diagnosed as a single tumor were collected and divided into control group and intervention group according to the presence or absence of preoperative facial nerve reconstruction by DTI.The two groups were compared in terms of improvement rate of facial nerve function, tumor resection degree, and operation duration. Results: A total of 32 cases were included in the control group,53.13% of the patients had excellent(House-Brackmann rating:?-?grade)facial nerve function after surgery, and compared with preoperative facial nerve function, the grade deteriorated accounted for 53.13%.What`s more, the tumor near/total excision rate was 84.38%,and the average surgical duration was 7.89±0.76 hours.Meanwhile, the intervention group had a total of 30 cases,76.67% of the patients had excellent facial nervefunction after surgery, and compared with preoperative facial nerve function, the grade deteriorated accounted for 10.00%,the tumor near/total excision rate was 93.33%,and the average surgical duration was 7.17±0.91 hours. Conclusion: Preoperative reconstruction of facial nerve by DTI is of great significance to the preservation of facial nerve function in patients with acoustic neuroma,by which the operator can intuitively observe the relative position relationship between the facial nerve and the tumor before the operation, and can identify and protect the facial nerve during the operation purposedly,so as to reduce the incidence of facial nerve injury.Which can also improve the retention rate of facial nerve function after operation and effectively shorten the operation time.
