Objective: To investigate the expression of NT-proBNP in patients with different types of acute coronary syndromes,and to further explore the significance of early detection of NTproBNP in patients with acute coronary syndromes to assess the occurrence of in-hospital ventricular arrhythmias.Methods: A total of 192 patients with acute coronary syndrome who were admitted to the Second People's Hospital of Gansu Province from June 2016 to October 2019 were studied.Among them,77 were acute myocardial infarction and 115 were unstable angina.71 patients with stable angina pectoris admitted at the same period were randomly selected as the control group.All patients underwent coronary angiography.According to the dynamic electrocardiogram,the study components were divided into ventricular arrhythmia group(98cases)and non-ventricular arrhythmia group(94 cases).The plasma NT-proBNP concentration was determined by immunofluorescence chromatography.The differences between the NT-proBNP levels of each group were compared.According to the level of NTproBNP,ACS patients were divided into NT-proBNP increased group(?300pg / ml)and NTproBNP normal group(<300pg / ml),the incidence of ventricular arrhythmia between NTproBNP increased group and NT-proBNP normal group was compared.Logistic regression was performed to analyze the diagnostic value of NT-proBNP for ACS and the predictive value of ventricular arrhythmias in the hospital.Results:1.NT-proBNP levels in patients with acute coronary syndrome[360.60(652.28)]were higher than those in control group[199.10(366.40)],the difference was significant(P = 0.000 <0.05).Logistic multifactors regression analysis showed that NT-proBNP was an independent risk factor for the occurrence of acute coronary syndrome(OR = 1.001,95%CI = 1.001 ~ 1.002,P=0.024<0.05).2.Plasma NT-proBNP levels gradually increased in the control group,UA,and AMI patients,and the differences between the three groups were statistically significant [199.10(226.40): 310.3(408.40): 544.4(1546.45),P < 0.017].3.The level of NT-proBNP in patients with ventricular arrhythmia group in ACS patients was higher than that in patients without ventricular arrhythmia group,the difference was statistically significant [ 461.85(1583.95): 303.60(364.88),P = 0.000 <0.05].4.The incidence of ventricular arrhythmia in patients with increased NT-proBNP in ACS(57.52%)was higher than that in patients with normal NT-proBNP(34.74%),(?2=10.75,P=0.001<0.05).In Logistic multivariate regression analysis,NT-proBNP was not an independent risk factor for ventricular arrhythmias in ACS patients.Conclusion:1.NT-proBNP is highly expressed in acute coronary syndrome and is an independent risk factor for acute coronary syndrome.2.The risk of ventricular arrhythmia in ACS patients may increase with the increase of NT-proBNP concentration.The NT-proBNP concentration should be detected immediately after admission to assess the risk of ventricular arrhythmia in the hospital.
