Objective:1.To investigate the characteristics of skin lesions in hemodialysis patients with chronic renal failure.2.To explore the influencing factors of skin lesions in hemodialysis patients with chronic renal failure.Methods:The incidence and clinical characteristics of lesions in 316 patients with chronic renal failure on hemodialysis were collected,and the relevant laboratory examination indexes were collected.Firstly,the influencing factors were screened by chi-square analysis and t test,and then the relevant risk factors were determined by Logistic regression analysis.Results:1.Among the 316 hemodialysis patients,248(78.48%)had lesions.106(33.54%)had two kinds of lesions,and 45(14.24%)had three or more kinds of lesions.There were 110(34.81%)cases of pruritus,231(73.10%)cases of xerosis cutis,81(25.63%)cases of pigmentation,64(20.25%)cases of nail lesions,41(12.97%)cases of skin infection,27(8.54%)cases of subcutaneous hemorrhage,and 4(1.27%)cases of calciphylaxis.2.Analysis of influencing factors:(1)Age,calcium-phosphorus product,parathyroid hormone and ?2-microglobulin were the independent risk factors for pruritus,and Kt/V was an independent protective factor for pruritus.(2)Dialysis age and blood phosphorus were the independent risk factors for xerosis cutis.(3)Dialysis age was an independent protective factor for pigmentation.(4)Serum calcium was an independent risk factor for nail disorder,and albumin was an independent protective factor for nail disorder.(5)Diabetic nephropathy ratio,age,dialysis age,blood calcium and parathyroid hormone in patients with onychomycosis were significantly higher than those without onychomycosis(P<0.05),and albumin and Kt/V were significantly lower than those without onychomycosis(P<0.05).(6)Age,dialysis age,blood calcium,parathyroid hormone and hemoglobin in patients with subcutaneous hemorrhage were significantly higher than those without subcutaneous hemorrhage(P<0.05),and Kt/V was significantly lower than those without subcutaneous hemorrhage(P<0.05).Conclusion:1.Hemodialysis patients with chronic renal failure may be complicated with one or more skin lesions,among which pruritus,xerosis cutis,pigmentation,nail disorder,skin infection and subcutaneous hemorrhage are common skin lesions.2.Age,calcium-phosphorus product,parathyroid hormone and ?2-microglobulin are the independent risk factors for pruritus,and Kt/V is an independent protective factor for pruritus.3.Dialysis age and blood phosphorus are the independent risk factors for xerosis cutis.4.Dialysis age is an independent protective factor for pigmentation.5.Serum calcium is an independent risk factor for nail disorder and albumin is an independent protective factor for nail disorder.Objective: 1.To evaluate the severity of pruritus in hemodialysis patients with chronic renal failure and analyze the influencing factors.2.To analyze the effects of pruritus severity on quality of life in hemodialysis patients with chronic renal failure.Methods: The visual analogue scale(VAS)and 5-D itch scale(5D-IS)were used to investigate the pruritus severity of patients with chronic renal failure during hemodialysis.The Short Form with 36 questions(SF-36)was used to investigate the quality of life of patients.Firstly,the influencing factors were screened by chi-square analysis and t test,and then the relevant risk factors were determined by Logistic regression analysis.Results: 1.Logistic regression analysis showed that :calcium-phosphorus product,parathyroid hormone and leukocyte count were the independent risk factors for moderate to severe pruritus.Kt/V was an independent protective factor for moderate and severe pruritus.2.Logistic regression analysis showed that :diabetic nephropathy ratio,moderate to severe pruritus(baseline),age and blood calcium were the independent risk factors for the quality of life.Conclusion: 1.Calcium-phosphorus product,parathyroid hormone and leukocyte count are the independent risk factors for moderate to moderate to severe pruritus.Kt/V is an independent protective factor for moderate and severe pruritus.2.Diabetic nephropathy ratio,moderate to severe pruritus(baseline),age and blood calcium are the independent risk factors for the quality of life.
