| Objective:To investigate the prevalence and treatment of anemia in maintenance hemodialysis(MHD)patients in Shanxi Province,and to study the influencing factors of hemoglobin target.Methods:Clinical data of maintenance hemodialysis patients reported online in Shanxi Province in 2017 were collected through the national blood purification registration system,including basic data,dialysis status,drug treatment status and laboratory indicators.Hemoglobin(Hb)≥110 g /L was taken as the target of treatment.The data collected were then sorted out and analyzed.Results:(1).In 2017,the average Hb level of MHD patients in Shanxi Province was107.2±19.2g/L,and 45.7% patients reached the target level.The utilization rate of erythropoietin was 97.6%,and that of intravenous iron was 35.5%.The gender stratification showed that males had a higher percentage of Hb≥110 g/L than females,age stratification showed that with the increase of age,the Hb target rate showed an increasing trend.(2).Compared with patients whose <110 g/L,patients with Hb≥110g/L set the higher proportions of male、average age、dialysis age、import dialyser uses、proportional urea rate and urea clearance score、intravenous iron、l-carnitine、bicarbonateion concentration 、 albumin and triglycerides,and lower proportions of the primary disease of hypertension 、 Percentage of patients with transferrin saturation ≤30% 、parathyroid hormone and c-reactive protein(all P<0.05).(3)The multi-factor logistic regression analysis showed that female、oral iron、high parathyroid hormone、high C-reactive protein and low serum albumin were risk factors for Hb targeting failure.Conclusion:The overall Hb target rate of MHD patients in Shanxi Province(Hb≥110g/L)is45.7%,still lagging behind that in developed regions.Rational use of intravenous iron agents,improvement of nutritional status and correcting of inflammatory reaction can improve the hemoglobin leves. |
