Objective:Through the retrospective analysis of the clinical data of elderly patients with multiple myeloma(multiple myeloma,MM),to explore the risk factors,prognosis and curative effect analysis of elderly patients with MM kidney damage,to provide an objective basis for the prognosis judgment and optimal treatment of elderly patients with MM kidney damage.Methods:The clinical data of 118 patients with initial elderly MM in our hospital from january 2011 to december 2018 were collected for analysis,divided into renal function impairment group(RI group)and normal renal function group(non-lateral group)to analyze their clinical characteristics and prognosis.the RI group was divided into combination group(which also included bortezomib,thalidomide chemotherapy regimen)and single group(which mainly included bortezomib or thalidomide chemotherapy regimen)to analyze the differences in renal response and survival benefits.Results:Univariate analysis showed that DS stage,pulmonary infection,uric acid,β2microglobulin,leukocytes were higher in RI group and hemoglobin was lower than innon-RI group(P<0.05).Multivariate logistic regression analysis showed that β2microglobulin was an independent risk factor for renal impairment in elderly MM patients.The OS and PFS of RI group were significantly lower than those of non-group(P<0.05)according to the method.The renal response of the combined group was significantly better than that of the single group(P<0.05),which could benefit the PFS of elderly patients with MM renal impairment(P<0.05).Conclusion:Elderly MM patients with renal impairment have poor prognosis.Prognosis can be improved by actively monitoring and controlling various risk factors and selecting chemotherapy regimen with bortezomib and thalidomide.
