Objective: Aortic valve bilobal malformation can be divided into longitudinal fissure type, transverse fissure type and oblique fissure type according to the closed shape of the valve leaflet. This article intends to explore the effect of different types of aortic valve bilobal malformation on valve function and aortic dilation. Methods: From November 2017 to February 2020,80 patients with bicuspid aortic valve malformation were selected from the Affiliated Hospital of Qinghai University.According to the shape of bicuspid aortic valve in the diastolic closure line, the patients were divided into longitudinal, transverse and oblique types. The diameter of the sinus and ascending part of the aorta, the peak velocity of aortic cross valve, the diameter of aortic regurgitation, the degree of valve regurgitation and stenosis, and the presence of calcification were measured. Finally, the clinical and echocardiographic data of the three groups were collected for statistical analysis.Results:(1)Of the 80 patients with aortic valve bilobal malformation, there were 30 cases in the oblique fissure group,28 cases in the transverse fissure group, and 22 cases in the longitudinal fissure group.(2)The incidence of aortic valve regurgitation in the oblique fissure group, transverse fissure group, and longitudinal fissure group was 76.67%,35.71%,and 22.73%,and the incidence of stenosis was 46.67%,21.43%,and 18.18%,respectively.The rates were 66.67%,39.29% and 18.18% respectively. The comparison of the incidence of reflux in the three groups showed that the incidence of the oblique fissure group was higher than that of the other two groups,P<0.017,the difference was statistically significant, while the three groups of patients The comparison between the incidence of stenosis,P>0.05,the difference was not statistically significant.Comparison of the incidence of calcification among the three groups showed that the incidence of oblique fissure group was higher than that of longitudinal fissure group,P<0.017,the difference was statistically significant.(3)The comparison of the degree of aortic valve regurgitation and stenosis among the three groups of patients,P>0.05,the difference was not statistically significant.(4)Comparing the inner diameter of the ascending aorta in the three groups, the inner diameter of the ascending aorta in the oblique fissure group was 41.17±4.81 mm,the inner diameter of the ascending aorta in the transverse fissure group was 38.86±3.58 mm,and the inner diameter of the ascending aorta in the longitudinal fissure group Is 34.00±2.99 mm,the diameter of the ascending part of the oblique fissure group is larger than that of the transverse fissure group and the longitudinal fissure group,P<0.017,the difference is statistically significant, compared with the longitudinal fissure group,P>0.05,no difference Learn meaning.Comparing the inner diameter of the aortic sinus in the three groups, the inner diameter of the aortic sinus in the oblique fissure group was 37.37±4.32 mm,the inner diameter of the aortic sinus in the transverse fissure group was 35.79±3.21 mm,and the inner diameter of the aortic sinus in the longitudinal fissure group was 34.36±.2.94 mm,the diameter of the sinus in the oblique fissure group is greater than that in the longitudinal fissure group,P<0.017,the difference is statistically significant, compared with the transverse fissure group and the transverse fissure group, and the transverse fissure group and the longitudinal fissure group,P>0.05,the difference is not statistically significant. Conclusion: 1.In the impact on aortic valve regurgitation, the oblique fissure type is more prone to valve regurgitation than the other two types.2.In the effect on aortic valve calcification, oblique fissure type is more likely to cause valve calcification than longitudinal fissure type.3.In the effect on the dilation of the ascending aorta, the oblique fissure type is more likely to cause the ascending aorta to widen than the other two types.4.In the impact on the expansion of the aortic sinus, the oblique fissure type is more likely to cause the aortic sinus to widen than the longitudinal fissure type.
