[Purpose] The purpose of this study is to explore the optimal timing of surgical intervention after neoadjuvant chemotherapy for advanced gastric cancer,which can not only effectively reduce the tumor staging,but also do not increase the incidence of postoperative complications.[Methods] 67 patients with advanced gastric cancer requiring neoadjuvant chemotherapy were collected.All patients were divided into observation group(4weeks ≤ T < 6 weeks)and control group(6weeks ≤ T < 8 weeks)according to different intervals between the last chemotherapy and surgical treatment.Both groups were treated with 2-4 cycles of DOF chemotherapy before surgery,and radical gastrectomy(D2)was selected as the surgical method.[Results] The rate of T downstaging and N downstaging in the observation group were respectively 70.6% and 61.8%,whcih were significantly higher than the control group(P<0.05).In the aspect of tumor regression,the proportion of TRG1 and 2 in the observation group was also higher than that in the control group.The difference was statistically significant(P<0.05).There was no statistically significant in the incidence of complications,such as anastomotic fistula,abdominal infection,pulmonary infection and abdominal bleeding(P>0.05).[Conclusion] In the clinical treatment of advanced gastric cancer,the treatment mode of preoperative chemotherapy combined with radical resection is effective and safe.The surgery within 4-6 weeks after the end of neoadjuvant therapy can achieve a higher rate of downstaging and tumor regression.Meanwhile,appropriate postponement of surgery is feasible and safe in clinical treatment.Even if the operation is delayed to 6 weeks later,it will not increase the incidence of postoperative complications.It?is?noteworthy?that this interval should not be too long.
