objective: To analyze the incidence of carotid and lower limb arterial plaque and its related risk factors in patients with type 2 diabetes mellitus.Methods:(1)Participants: A total of 6000 patients with type 2 diabetes who were hospitalized in hefei first people's hospital from January 2010 to December 2016 were collected in this study.3065 patients to rule out the reason as follows: physical examination,clinical parameters and auxiliary examination results is not complete,severe liver and kidney dysfunction of diabetes,acute complications of diabetes,ultimately 2935 patients were included in this study,including 1571 men and 1364 women.The mean age was 59.6±12.4 years old.(2)Data entry method: Epida database was established and grouping was used.The first group was used to input patient's basic information including their name,age,gender,smoking history,drinking history,family history,diagnosis and whether they had diabetes complications.The laboratory test items of the second group included total totol cholesterol(TC),total triglyceride(TTG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FPG),serum uric acid(SUA),fasting c-peptide,and glycosylated hemoglobin(Hb A1c).The imaging examinations data of the third group included ECG,cranial CT or MRI,carotid artery and lower extremity artery color doppler ultrasound.(3)2935 inpatients with type 2 diabetes mellitus were studied retrospectively.Firstly,according to the results of Doppler ultrasound of carotid artery and lower limbs,the patients were divided into three groups: non-plaque group(group A,n = 655),and non-plaque group(group A,n = 655).There were 1044 cases of simple carotid or lower extremity atherosclerotic plaque group(group B)and 1236 cases of combined carotid and lower extremity atherosclerotic plaque group(group C).The clinical data and biochemical indexes were compared among the three groups,and the prevalence of cardio-cerebrovascular events was compared among the three groups.Secondly,the risk factors that might affect the formation of plaque in Arteria congee were analyzed by multivariate Logistic and then entered into the regression equation.The factors of atherosclerotic plaque were analyzed by stratified analysis.Results:(1)the incidence of carotid and lower extremity artery plaque in diabetes mellitus was77.7%.(2)The age,duration of diabetes,systolic blood pressure,serum creatinine and uric acid in the non-plaque group were lower than those in the other two groups,while fasting blood glucose was significantly higher in the non-plaque group than in the other two groups(P < 0.05).Compared with non-plaque group,carotid artery and lower extremity atherosclerotic plaque group were older,longer course of disease,higher serum creatinine,LDL-C,uric acid,diastolic blood pressure,body mass index and TTG,e GFR were lower than those of non-plaque group(P < 0.05).(3)The prevalence of hypertension and CBVEs in the two plaque groups was significantly higher than that in the non-plaque group(P < 0 05).The prevalence of hypertension,diabetic foot and CVEs,CBVEs in patients with carotid artery and lower extremity artery combined with plaque was significantly higher than that in non-plaque group(P < 0.05).The prevalence of CVEs,CBVEs in patients with carotid artery and lower extremity artery combined plaque group was higher than that in simple plaque group,and the difference was statistically significant(P < 0 05).Compared with the non-plaque group,hypertension and CBVEs in the simple plaque group were significantly higher than those in the non-plaque group.The difference of morbidity was statistically significant(P < 0.05).(4)The incidence of diabetic vascular plaque was correlated with age(OR=1.106,P=0.000),course of disease(OR=1.004,P=0.000)and sex(OR=0.338,P=0.000),BMI(OR=0.965,P=0.011).Conclusion: The prevalence of peripheral atherosclerotic plaques is high in type 2diabetic patients.The formation of diabetic atherosclerotic plaques is associated with age,course of disease,sex and BMI.
