Objective:To fully understand the relationship between heart failure HF and obstructive sleep apnea OSA,in order to identify OSA patients in time and accurately,and give symptomatic treatment.Methods:From July 2017 to July 2019,160 patients with heart failure were enrolled in the First Affiliated Hospital of Shanxi Medical University.Polysomnography PSG was performed.According to the results of sleep detection,160 patients were divided into two groups,80 in each group.Group A was OSA with heart failure(AHI ? 5 times/ h),defined as the observation group.Group B was defined as the control group(AHI < 5 times / h).General data(including height,weight,age,gender,history of hypertension,history of tobacco and alcohol,blood lipid,uric acid,n-terminal-b-type natriuretic peptide precursor,etc.)were collected and 24-hour Holter and echocardiography were performed.Relevant indexes were analyzed according to whether there was OSA in the two groups.Results:A total of 160 patients were included,including 110 males and 50 females,aged24-84 years,with an average age of(60.39 ± 14.93)and a body mass index of(26.78 ± 5.05)kg / m2.There was no significant difference in sex,age,incidenceof hypertension and hyperlipidemia between patients with heart failure and OSA group and those with simple heart failure group(P > 0.05).BMI index [P = 0.006,95% CI(1.104,1.798)] left ventricular ejection fraction(LVEF)[P = 0.033,95% CI(0,0.536)],left ventricular end diastolic diameter(lved)[P = 0.017,95% CI(1.03,1.358)],right ventricular end diastolic diameter(rvedv)[P = 0.003,95% CI(1.174,2.165)],uric acid(UA)[P = 0.019,95% CI(1.002,1.02)and n-terminal-b natriuretic peptide precursor(NT proBNP)[P = 0.029,95% CI(1,1.001)],there was statistical significance(P < 0.05).There was statistical significance(P < 0.05).BMI,uric acid,NT proBNP in group A were higher than those in group B,and LVEF in group A was higher than those in group B The heart rate variability(HRV)of patients in group A was significantly lower than that in group B.after adjusting the risk factors of smoking and drinking history,BMI index,NT proBNP,etc.,the results showed that long-term drinking history,hyperuricemia,increased NT proBNP level,decreased left ventricular ejection fraction,increased ventricular volume and decreased HRV were HF patients with OSA Risk factors.Conclusion:For obesity,serious hyperuricemia,arrhythmia and heart failure patients with serious impairment of cardiac function,we should be alert to the occurrence and development of OSA,and HF patients with OSA have serious cardiac function impairment,which needs early intervention to prevent adverse events.
