Object:To observe the distribution of hyperhomocysteinemia(HHcy)in patients with mild cognitive impairment(MCI)and the relationship between HHcy and TCM syndrome types,so as to formulate specific prevention and treatment plans for MCI patients and provide evidence for TCM early intervention in dementia. Methods:The subjects of this study are all outpatients and inpatients in the Department of integrated traditional Chinese and Western medicine of Affiliated Hospital of Hebei University.Based on the inclusion, exclusion and rejection criteria,MCI patients who meet the criteria are finally selected.According to blood homocysteine levels, they are divided into three groups: mild hyperhomocysteinemia, moderate hyperhomocysteinemia, and severe hyperhomocysteinemia.Refer to the expert group on Prevention and control of cognitive function in 2006,at the same time, according to the scale for the differentiation of syndromes of vascular dementia developed by the professional committee of anti-aging of internal medicine of Chinese Academy of traditional Chinese medicine(SDSVD)to draw up TCM syndrome differentiation and classification of MCI,to study the correlation between general data(gender, education, age),risk factors and different levels of hyperhomocysteinemia in patients with MCI,the relationship between different levels of homocysteine and MMSE,MOCA scores, and the correlation between TCM Syndrome Types and different levels of homocysteine.Spss22.0 is selected as the statistical software for data analysis,in which frequency method or percentage method is used for counting data, mean plus minus standard deviation is used for measuring data, chi square test and one-way ANOVA are used for statistical analysis of data, and all test standards are statistically significant at the level of P < 0.05.Results:1.general factor: According to Chi square test,p>0.05,there was no significant difference in the distribution of sex,age and education level of MCI patients among the three groups with mild, moderate and severe HHcy.2.Risk factors: According to Chi square test,p<0.05,the risk factors of MCI patients(hypertension, diabetes, coronary heart disease, dyslipidemia)were significantly different in the distribution of mild, moderate and severe HHcy.3.MOCA cognitive assessment scale: By one-way ANOVA,p<0.05 showed that the difference between groups was statistically significant. It was concluded that the MOCA score of MCI patients was significantly different among the three groups of mild, moderate and severe HHcy.4.MMSE integral check scale: By one-way ANOVA,p<0.05 showed that the difference between groups was statistically significant. The MMSE score of MCI patients was compared among the three groups of mild, moderate and severe HHcy,and the difference was statistically significant.5.Correlation between TCM syndromes and HHcy level in MCI patients: According to one-way ANOVA,p<0.05 indicates that there is statistical significance in the differences between groups. It is concluded that there is statistical significance in the differences between the three groups of mild, moderate and severe HHcy in different TCM Syndromes of MCI patients.6.Occurrence rate of multiple syndromes in MCI patients: According to the comparison of rates, the three groups of MCI patients with mild, moderate and severe homocysteinemia are mainly of complex syndrome type, and the single syndrome type is rare.Conclusion:1.Among MCI patients, hypertension, diabetes, coronary heart disease, dyslipidemia and other risk factors are closely related to mild to severe HHcy,among which hypertension is related to mild HHcy,diabetes is related to moderate to severe HHcy.2.In patients with MCI,MMSE and MOCA scores were negatively correlated with HHcy in different degrees.3.There is a correlation between the TCM syndrome types of MCI patients and different levels of HHcy,that is,in mild HHcy,the TCM syndrome types of MCI patients are mainly blood stasis and kidney essence deficiency;in moderate HHcy,the TCM syndrome types of MCI patients are mainly kidney essence deficiency and phlegm turbid obstruction of orifices;in severe HHcy,the TCM syndrome types of MCI patients are mainly blood stasis and phlegm turbid obstruction of orifices. In addition, the three groups of patients mostly have complex syndromes, but a single syndrome is rare.
