The Association Of EZH2 With Clinicopathological Features And Efficacy Of Neoadjuvant Chemotherapy In Breast Carcinoma

Abstract/Summary:
Objective: To analyze the clinical and pathological data of 80 patients with breast carcinoma who receiving neoadjuvant chemotherapy,detect the expression of EZH2 in cancer tissues before and after neoadjuvant chemotherapy retrospectively,and investigate the relationship between EZH2,the clinicopathological features of breast carcinoma and the efficacy of neoadjuvant chemotherapy.Methods: Collecting clinical and pathological data of 80 patients with invasive breast ductal carcinoma who received neoadjuvant chemotherapy in the department of tumor surgery,affiliated Hospital of Qinghai University from December 2014 to December 2018,and recording their ages,tumor sizes,tumor stages,lymph node metastasis,tissue credits and molecular typing.The expression of EZH2 in breast carcinoma tissues before and after neoadjuvant chemotherapy was detected by Immunohistochemistry.The relationship between EZH2,general clinicopathological characteristics of breast carcinoma and the efficacy of neoadjuvant chemotherapy was analyzed.SPSS 24.0 software was used for statistical analysis of the data.Results:1.Among the 80 cases of invasive breast ductal carcinoma,56 cases were positive for EZH2(70.00%),and the difference between expression of EZH2,tumor site,histological grade and breast carcinoma molecular typing was statistically significant(p < 0.05).The positive rate of EZH2 in the lymph node metastasis group(84.38%)was higher than that in the non-lymph node metastasis group(60.4%).The highest positive rate of EZH2 was Histologic grade Ⅲ(85.7%),EZH2 in triple negative subtype(93.7%)and HER2 over expression subtype(81.8%)were higher than that of Luminal subtype.There was no significant difference with age,nationality,menopausal status,location of tumors,tumor size and TNM stage(p > 0.05).2.In breast carcinoma tissues before neoadjuvant chemotherapy,the expression of EZH2 was significantly different among ER,PR,HER2 and Ki67 subgroups.3.Of the 80 patients,7 achieved pathologically complete remission(p CR)(8.75%).The expression of EZH2 in breast cancer tissue before neoadjuvant chemotherapy was significantly different among the subgroups of neoadjuvant chemotherapy(p<0.001),With the increase of the positive rate of EZH2,the remission degree of neoadjuvant chemotherapy increased(Gamma= 0.712,p<0.001).4.The expression of ER,HER2 and Ki67 and the efficiency of neoadjuvant chemotherapy were significantly different(p < 0.05).The effective rate of triple negative chemotherapy: triple negative subtype(81.3%)> HER2 over expression subtype(72.7%)>Luminal B subtype(59.4%)> Luminal A subtype(30.0%),p < 0.05.The difference was statistically significant.There were no significant differences between the expression of P53 and the efficacy of neoadjuvant chemotherapy(p > 0.05).Conclusion:1.The expression of EZH2 was associated with lymph node metastasis,the histological grade,the expression of ER,PR,HER2,Ki67 and molecular typing of breast carcinoma.The expression rate of EZH2 was the highest in lymph node metastasis group,histological grade Ⅲ group and triple negative breast carcinoma.2.The expression of EZH2 in breast carcinoma before neoadjuvant chemotherapy was related to the efficacy of neoadjuvant chemotherapy,and the remission degree of neoadjuvant chemotherapy increased with the increase of the positive rate of EZH2,suggesting that patients with positive EZH2 were more sensitive to chemotherapy.EZH2 has a certain reference value in predicting the response to neoadjuvant chemotherapy.3.The expression of ER,HER2,Ki67 and breast carcinoma molecular typing was related to the efficacy of neoadjuvant chemotherapy in breast carcinoma tissue before neoadjuvant chemotherapy.It suggests that they can provide some reference in predicting the efficacy of neoadjuvant chemotherapy.
