Purpose:The purpose of this study was to investigate the relationship between the levels of trace elements zinc and magnesium and the progression of diabetic nephropathy in patients with type 2 diabetes.Methods:We collected the clinical data of the patients with type 2 diabetes mellitus(T2DM)and diabetic nephropathy(T2DN)who were hospitalized in the first Hospital of Jilin University,At the same time,selected other persons check-up at physical examination center.The above results were analyzed to explore the predictors of T2 DN and the correlation between the level of zinc and magnesium and the progression of T2 DN.Results:1.According to whether the patient had T2 DM or T2 DN,they were divided into three groups: normal control group(N = 20),T2 DM group(N = 24),T2 DN group(N = 59).Comparing the clinical characteristics of the three groups,it was found that serum zinc(OR:0.295,95%CI: 0.111-0.784,P<0.05),fasting blood-glucose(OR: 3.018,95%CI: 1.106-8.239,P<0.05),IL-10(OR: 12.445,95%CI: 1.552-19.672,P<0.05)were predictors for DN.2.T2 DN patients were divided into four subgroups: diabetic normal proteinuria group(N = 24),microproteuria group(N = 18),massive proteinuria group(N = 24)and renal failure group(N = 17).Four groups of biochemical indexes were compared.It was found that with the progression of T2 DN,the serum zinc,urine zinc and urine magnesium decreased,IL(interleukin)-6 and IL-8 increased.3.Analyzing the correlation between inflammatory factors and trace elements in patients with T2 DN,we found that serum zinc and urine zinc were negatively correlated with serum IL-6 and IL-8.Conclusions:1.The serum levels of zinc and magnesium in T2 DN patients were significantly lower than those in healthy control group,serum zinc level are the predictors of T2 DN and as an independent protective factor for T2 DN.2.With the progression of T2 DN,the decrease of serum levels of zinc,urine zinc and urine magnesium is aggravated.Hypozincemia may promote progression of T2 DN by exacerbating inflammatory responses.
