| Object:To analyze the related factors for neovascular glaucoma(NVG)after vitrectomy in eyes with proliferative diabetic retinopathy(PDR).Methods:Clinical data of 34 PDR patients(34 eyes)with NVG after vitrectomy in our hospital from January 2012 to December 2018 were retrospectively analyzed as the experimental group.Among the 3088 PDR patients without NVG after vitrectomy during this period,200 patients(200 eyes)were randomly selected and the clinical data were analyzed as the control group(12 of them did not meet the inclusion criteria and a total of 188 eyes were included).Detailed record patient information,including patient sex,patient age,creatinine levels,fibrinogen levels,history of hypertention,history of preoperative panretinal photocoagulation or intravitreal anti-VEGF agent,preoperative pseudophakia,diabetic retinopathy preoperative staging,combined phacoemulsifification with vitrectomy,intraoperative retinal tamponade use,repeat surgery for DR,etc.R software was used for statistical analysis and the establishment of a clinical predictive model.Results:1.Compared with women,the incidence of postoperative NVG is higher in men.2.Patients with preoperative pseudophakia have a high incidence of NVG after surgery.3.The incidence of postoperative NVG was high in patients who underwent repeat surgery for DR.4.The higher the DR staging,the higher the incidence of postoperative NVG.5.The model was established with gender,age,repeat surgery,DR staging,preoperative crystal state,preoperative anti-vegf therapy,and cataract surgery as predictors.The c-index value of the model was 0.769,AUC=0.764,and the adjusted c-index value was 0.718.Decision curve analysis showed that the nomogram was clinically useful when intervention was decided at the possibility threshold of 2%-86%.Conclusion:1.Male,preoperative pseudophakia,repeat surgery and high DR stages are important risk factors of postoperative NVG after vitrectomy in PDR patients.2.The model was established with gender,age,repeat surgery,DR staging,preoperative crystal state,preoperative anti-vegf therapy,and cataract surgery as predictors,which was significant for the prediction of NVG after PDR vitrectomy. |
