| Objective:This article compared the efficacy of autograft and allograft surgery of thoracic and lumbar tuberculosis via posterior approach. Methods:We retrospectively studied 41 patients with thoracic or lumbar tuberculosis who underwent surgery via the posterior approach debridement and fusion with instrumentation. The autograft group(Group A)consisted of 24 patients and the allograft group(Group B)consisted of17 patients.Assessment included operation duration, blood loss,ASIA,VAS,kyphotic angle(preoperatieve, postoperative and final follow-up),kyphosis correction angle and correction loss, time of fusion. Results:Every patient achieved fusion. The time of fusion was significantly earlier in the autograft group(6.54±0.98 weeks)compared to the allograft group(8.17±1.59 weeks).Operation duration and blood loss of allograft group(219.25±26.45 min and 522.08±133.05ml)was less than that of the autograft group(188.70±29.11 min 和 390.59±149.81ml),but there was no statistical significance. Both groups had similar kyphosis correction,hospital-stay time and follow-up time. The kyphotic angles loss of correction were also insignificant between two groups.VAS amelioration was 2.04±1.23(Group A)and 2.18±1.07(Group B),whichwas insignificant in both groups. Conclusion:Both autograft and allograft can achieve satisfactory fusion and kyphosis correction. The autograft leads to faster fusion compared to the allograft, which is significant(P < 0.05).However,allograft avoids the donor site complications and with sufficient supplement bone graft. Therefore, allograft could be an alternative in the treatment of single thoracic and lumbar tuberculosis. |
