| Objective By detecting serum homocysteine in different population urinary inhibition(homocysteine,Hcy)and C(Cys C cystatin C)levels,and to explore the levels of these two factors and primary hypertension cerebral hemorrhage and its the correlation of disease severity,whether can pass the level of the two factors to predict the occurrence of primary hypertension cerebral hemorrhage,or rough estimated blood loss,and degree of nerve defect;It provides a new method for clinical monitoring and prevention of hypertensive intracerebral hemorrhage and scientific and theoretical basis for drug research and development of prevention of essential hypertensive intracerebral hemorrhage.Methods In September 2017-January 2020 collected during the second hospital affiliated to medical school in shenyang cardiology,73 cases of patients with essential hypertension(eh),neurosurgery 27 cases of hypertensive cerebral hemorrhage patients(22 cases aneurysm and arteriovenous malformation in 5 cases),61 patients with primary hypertension cerebral hemorrhage(blood loss less than 30 ml of 29 cases of patients,the bleeding more than 30 ml of 32 cases of patients,eliminate hemorrhage breaking into the ventricles,and were bleeding on the screen),70 patients with healthy controls,patients with cerebral hemorrhage were first onset.Record all basic information of the patient(including the patient’s admission number,name,sex,age,height,weight,permanent residence,nature of work,etc.);The patient was questioned and other relevant medical history was excluded.The amount of blood loss of patients with cerebral hemorrhage was calculated based on CT images,and the values of related indicators in blood,including Hcy,Cys C,blood lipid,blood glucose,cholesterol,blood routine,troponin,and cardiac natrium,were recorded through clinical test results(no more than 24 hours are required from the onset to the collection of blood).At the time of admission,patients with cerebral hemorrhage were examined,including consciousness,language,eye opening,muscle strength,movement,etc.,and GCS score was performed.SPSS23.0 statistical software was used to analyze the data,and the correlation between serum concentrations of Hcy and Cys C and patients with essential hypertensive cerebral hemorrhage was analyzed.The relationship between serum Hcy and Cys C concentration,GCS score and blood loss was analyzed linearly.T test was used for data conforming to normal distribution,non-parametric test was used for data not conforming to normal distribution,2 test was used for dichotomous variables,variance analysis was used for data of three or more groups,and logistic regression was used for multivariate analysisResults1.There was no statistical significance in the comparison of general data between the healthy control group,the essential hypertension group and the hypertensive cerebral hemorrhage group(P>,0.05).2.Serum levels of Hcy and Cys C in the essential hypertension group and the hypertensive intracerebral hemorrhage group were higher than those in the healthy control group,and the increase was more significant in the hypertensive intracerebral hemorrhage group(P<0.05).3.There was no significant difference in serum Hcy and Cys C levels between the non-hypertensive cerebral hemorrhage group and the healthy control group and the essential hypertension group(P>,0.05).4.In the hypertensive cerebral hemorrhage group,the serum levels of Hcy and Cys C in the group with blood loss greater than 30 ml were significantly higher thanthose in the group with blood loss less than 30ml(P<0.05).5.Serum levels of Hcy and Cys C in patients with hypertensive cerebral hemorrhage were positively correlated with the amount of blood loss(r=0.330,0.591,P<0.05)and negative phase with GCS score(r=0.290,0.261,P<0.05).6.Serum levels of Hcy and Cys C are independent risk factors affecting the amount of intracerebral hemorrhage in essential hypertension.Conclusion Serum levels of Hcy,Cys C in the essential hypertension complicated by cerebral hemorrhage has certain prediction value,therefore,when in the hypertensive patients serum Hcy,increased levels of Cys C should pay attention to the occurrence of cerebral hemorrhage,so as to achieve the purpose of monitoring condition,plasma Hcy,Cys C level was positively correlated with hypertension cerebral hemorrhage patients with blood loss,and negatively correlated with GCS score,by serum Hcy,Cys C level roughly predict the size of the hypertension cerebral hemorrhage volume and the severity of the disease;Hcy and Cys C levels are independent risk factors affecting the amount of hypertensive intracerebral hemorrhage,providing a scientific basis for the prevention and treatment of hypertensive intracerebral hemorrhage by reducing Hcy and Cys C values in the next step. |
