Objective:To analyze the risk factors of gastroesophageal reflux disease(GERD)in the elderly by questionnaire investigation,and to analyze the high risk factors of GERD in order to improve the quality of life(QOL)of the elderly.Methods: 91 GERD patients,64 males and 28 females,aged 60-93(70.29±9.74)years,were selected from October 2017 to November 2018.In addition,83 patients with chronic gastritis,70 males and 13 females,aged 60-95 years(74.86 ±10.55),were selected as control group.Age,sex,complications [e.g.hypertension,diabetes,coronary heart disease,hyperlipidemia,chronic obstructive lung,lumbar disc herniation,fatty liver,hyperuricemia,constipation),body mass index(BMI),] were recorded in both groups.Lifestyle(such as smoking history,drinking history,high-fat diet,sweet food,preference for strong tea,preference for coffee,bedtime diet,high-fat diet),drug use history.The patient was examined by gastroscopy for esophageal hiatal hernia,bile reflux,and duodenal ulcer.The RDQ scale was used to evaluate the gastroesophageal reflux disease,the frequency of attack and the severity of symptoms,and the psychological status of the patients was evaluated by the(HAD)scale of hospital anxiety and depression scale.Results: 1.In univariate analysis,sex,BMI,bile reflux,esophageal hiatal hernia,duodenal ulcer,nitrate drugs,constipation,high-fat diet,preference for sweet food,bedtime diet were found in GERD group and non-GERD group.There were significant differences in sleep disorder,anxiety/depression disorder and sleeping pills(P<0.05),and the percentage of these factors was significantly higher than that in non GERD group,so these factors could be considered as risk factors of GERD.2.In multivariate Logistic regression analysis,Logistic regression was used to analyze the single factor(sex,BMI,bile reflux,esophageal hiatus hernia,duodenal ulcer,nitrate,constipation,high-fat diet,sweetmeat).bedtime diet,sleep disorders,anxiety/depression disorders,taking sleeping pills,results showed,BMI(OR = 4.151,95%CI=1.270-13.572,P=0.019),bile reflux(OR=8.276,95%CI=2.460-27.840,P=0.001),duodenal ulcer(OR=4.087,95%CI=1.305-12.798,P=0.016),hiatal hernia(OR=4.566,95%CI=1.286-16.212,P=0.019),constipation(OR=4.113,95%CI=1.689-10.015,P=0.002),high fat diet(OR=8.537,95%CI=3.411-21.367,P=0.000),these factors can be considered as independent risk factors of GERD.Conclusion: Sex,BMI,bile reflux,esophageal hiatus hernia,duodenal ulcer,nitrate ester drugs,constipation,high-fat diet,preference for sweet food,bedtime diet,sleep disorder,anxiety/depression disorder,sleeping pills are risk factors of GERD.BMI,bile reflux,esophageal hiatus hernia,duodenal ulcer,constipation and high fat diet were independent risk factors for GERD.Psychological intervention can be used to assist GERD treatment by improving lifestyle.
