Application Of Case Management Model In Family Members Of Patients In ICU

As a special group in the hospital,intensive-care unit patients have a relatively long stay in the hospital,a serious degree of disease,and a relatively high medical cost,and they are the main concern of health care workers.The families who wait outside the intensive-care unit face enormous psychological,physical and financial challenges,but they have no dedicated staff to take care of them This group has not received enough attention.Case management is a social work model that provides integrated services to individuals and families with multiple problems and needs.Compared with traditional case work,it can connect multiple resources and provide more comprehensive services to case owners.This study selected 10 family members of patients from a children’s Hospital in Shanghai as the research object,and analyzed the concrete implementation process of case management,in order to explore the application effect and advantages of this model in medical social work This paper studies how to apply the methods of evaluation,coordination and resource link in the process of case management to help case owners solve practical problems.It is found that the case management service model can help the case owners to solve the practical problems in their lives,effectively build and strengthen the social support network of the case owners,and improve their ability to use resources and the overall quality of life in the hospital.In addition,the study shows that the case management mode also has obvious advantages in family members of patients in intensive care unit.First,it can further improve the one-to-one service of case work;second,it can integrate multiple resources to improve service efficiency;third,it can better train the management ability of social workers and improve service level.At the same time,in view of the shortcomings of the case management model,this paper puts forward Suggestions to constantly reflect on the improvement of the case management service system,link multiple resources to improve the integration of social resources,and strengthensocial work education and publicity to improve professional recognition,which is conducive to better solve the problem of family members of patients in intensive care unit in the future.
