A Comparative Study On The Effect On The Treatment Of Zygomatic Complex Fracture Based On Different Incision Selection

Objective:The curative effect,functional and aesthetic results was evaluated and analyzed by choosing different incision on zygomatic complex fractures for treatment in order to provide a basis for clinical work.Methods:Treatment on 72 patients with unilateral closed zygomatic complex fractures was evaluated in the First Hospital of Jilin University from October 2017 to October 2019.These samples were divided into two groups based on the different choices of incision:Group I: Patients that take the treatment on coronal incision,subciliary incision and vestibular sulcus incision.Group II: Patients that take the treatment on small incision upper zygomatic arch,subciliary incision,vestibular sulcus incision and lateral brow incision.We reviewed the medical records of these patients in the followed 6months postoperative,evaluated and compared the difference of the operating time and bleeding volume,postoperative length of stay,recovery of the fracture,complications,satisfaction rate between the two groups.The every individual fact of the operating time,bleeding volume,postoperative length of stay,recovery of the fracture,complications andsatisfaction rate between the two groups of patients was compared and evaluated according to the medical records of those patients in the followed 6 months.Results:1.The general medical records show: the surgery time was144.23±14.41 min,the bleeding volume was 138.81±11.89 ml,and postoperative length of stay was 10.79±2.96 d for group I.And the corresponding values of group II were 126.52±13.47 min,97.82 ±10.57 ml,and 8.27±3.24 d,respectively.2.The results on fracture reduction show: After surgery,facial appearance was symmetrical,no obvious malformation was observed and fracture reduction outcome showed no significant difference between the groups(P>0.05)according to the CT results.There were no sights of delayed union,nonunion,malunion and rejection for both groups.3.The results on related complications shows: Group I incision related types and incidence of complications is higher than group II.There were no obvious facial nerve injury.Limitation of mouth opening disappeared for a time after surgery.4.The results on satisfaction rate shows: The patients and surgeon satisfaction rate(93.10%,86.21%)in group II were both higher than those in group I.Conclusions:1.Coronal incision will serve well for various types of zygomatic complex fractures especially ones with obvious displacement,severe dislocation or comminuted fracture of zygomatic arch.2.Small incision upper zygomatic arch is capable of injuries lessened and reduction when zygomatic complex fracture with single line or adjacent double line fractures of zygomatic arch with no obvious displacement occurs.
