Objective:In this study,the level of Egr-1 in the serum of patients with type 2 diabetic retinopathy(DR),patients with type 2 diabetes mellitus(T2DM)and healthy people were measured,and the correlation and clinical significance between Egr-1 and Dr were analyzed to provide a theoretical basis for the clinical diagnosis and treatment of Dr.Methods:From January 2018 to April 2019,84 T2 DM patients in the Department of endocrinology of Jiuquan people's hospital were selected and divided into four groups according to the diagnosis of retinopathy(DR)by color fundus photography after mydriatic ophthalmoscope,including 44 cases in T2 DM group,40 cases in T2 DM combined with retinopathy group,and 41 cases in physical examination center of our hospital in the same period in the control group The sex and age of the healthy persons matched the case group.The level of Egr-1 in serum samples was determined by enzyme-linked immunosorbent assay(ELISA).The liver and kidney function,uric acid,fasting blood glucose(FBG),2-hour postprandial blood glucose,total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL)and high density lipoprotein(HDL)were measured,HDL)and other biochemical indicators,24-hour urine was collected to measure 24-hour urinary protein and Microalbumin levels,and fasting insulin(fins),HbA1 c and hs CRP levels were measured.HOMA-IR and glomerular filtration rate(EGFR)were calculated based on clinical data.All data were statistically analyzed with spss19.0 software,P < 0.05,which was considered statistically significant.Results:1.The serum Egr-1 level of T2 DM group and Dr group was significantly higher than that of the control group,the difference was statistically significant(P < 0.05).Compared with the control group,the levels of FBG and HbA1 c in the two groups were significantly higher,and there was a statistical difference between the two groups(P < 0.05),while there was no difference between the other related indexes(P > 0.05).The level of hs CRP in Dr group was significantly higher than that in control group(P < 0.05).According to the results of ANOVA,there were significant differences in 24 h urinary protein,24 h urinary microalbumin and EGFR in different Dr stages(P < 0.05),but there was no significant difference in Egr-1(P > 0.05).2.The correlation between Egr-1 and other factors was analyzed by Pearson correlation analysis.The results showed that Egr-1 was positively correlated with course of disease,age,systolic pressure,24 h urinary protein,24 h urinary microalbumin,bun,Cr,FBG,HbA1c%(r = 0.303,0.309,0.267,0.388,0.511,0.401,0.339,0.182,0.280,all P < 0.05).3.Single factor analysis showed that age,course of disease,Egr-1,HbA1 c were related to Dr.logistic regression analysis showed that Egr-1 was independently related to Dr and was a risk factor of DR(or = 1.205,95% CI,1.06-1.54,P = 0.005).4.The results of ROC analysis showed that the area under the ROC was 0.913(95% CI 0.854-0.972,P = 0.000),which indicated that Egr-1 had a certain accuracy in the diagnosis of Dr.Conclusions:1.The level of Egr-1 in the serum of Dr patients increased significantly,which may be a new inflammatory biomarker of DR,and may also be the target of Dr treatment in the future.2.Egr-1 is a risk factor of Dr.Egr-1 may be involved in the pathogenesis of Dr by activating and regulating the expression of inflammatory factors.
