Object:In order to improve the clinical treatment rate of Lumbar Disc Herniation(LDH),by studying the behavioral changes of LDH chronic lower extremity pain model rats,and based on the ERK-CREB-BDNF signaling pathway,preliminary study on Duzhong Yaotong pills for chronic LDH The mechanism of action of rats with lower limb pain provides objective laboratory data support for clinically improving the efficacy of LDH treatment and revealing the pharmacological research of Duzhong Yaotong Pills.Methods: Experiment 1: 24 male SPF SD rats,6-8 weeks old,body weight(250±20)g,were divided into normal group,sham operation group and model group.8 animals per group.The normal group did not do any treatment.The model group used the autologous nuclear transfer method to establish a rat model of lumbar disc herniation with reference to the literature.The sham operation group did not remove the nucleus pulposus and did not produce compression.For the three groups of rats,1d before modeling,2d after modeling,7d,14 d,21d,and 28 d after modeling were the observation time nodes.Through the measurement of gross behavior,heat pain threshold,and mechanical pain threshold,the chronicity of LDH rats was studied.The timing and mechanism of lower limb pain.Experiment 2: 72 male SPF SD rats,6-8weeks old,body weight(250±20)g,were divided into blank control group,model control group and treatment group according to the rat body weight number and random number table method.8 animals per group.The model control group and the treatment group were modeled by autologous nucleus pulposus transplantation.The treatment group was given gavage of Duzhong Yaotong pill solution,and the other two groups were given gavage of normal saline.The materials were collected 1d before administration,14 d after administration,and 28 days after administration.The protein expressions of ERK1,ERK2,CREB,and BDNF in rat hippocampus tissue were detected by Real-time PCR and Western blot.Results : Experiment 1:(1)The autologous nucleus pulposus transplantation method was successfully used to establish the rat model of LDH chronic lower extremity pain,and 14 days after the establishment of the model,the model group rats changed from acute postoperative acute pain to chronic pain,and chronic lower extremity pain was produced and maintained At a stable level;(2)ERK1,ERK2,CREB,and BDNF protein ratios in the hippocampus of thetreatment group were increased.After Duzhong Yaotongwan intervention treatment,the protein expression in the hippocampus of the treatment group was increased.(3)Duzhong Yaotong Pill has a positive therapeutic effect on LDH chronic lower limb pain model rats.Conclusion:(1)After the model of autologous nucleus pulposus transplantation was used for LDH modeling,pain and hyperalgesia appeared in the right hind limb,and the thermal pain threshold and mechanical pain threshold decreased significantly.After 14 days,the related proteins in the hippocampus of the model group were detected.Significantly decreased,the model of LDH chronic lower limb pain rats was successful.(2)Duzhong Yaotong Pill can significantly improve the pain behavior of LDH rats and increase the protein expression of ERK1,ERK2,CREB,and BDNF in hippocampus.It is speculated that Duzhong Yaotong Pill may play a role in suppressing pain through the ERK-CREB-BDNF signaling pathway The role of its analgesic effect may be one of the mechanisms of Duzhong Yaotong pills in the treatment of LDH.
