Clinical Effect Analysis Of Cementless Long Femoral Stem In The Treatment Of Periprosthetic Femoral Fracture

Object: To explore the difference of clinical efficacy between cementless long stem femoral prosthesis and cemented long stem femoral prosthesis in the treatment of Vancouver type B2 and B3 femoral periprosthetic fracture after THA.Method: This is a retrospective case-control study.According to inclusion and exclusion criteria,we chose 16 patients with cementless long-stem femoral prosthesis(experimental group)and 12 patients with cemented long-stem femoral prosthesis(control group)in our hospital from January 2013 to July 2018.To explore the difference of Harris score and Charnley score between the two groups of patients one year after revision surgery,and according to the postoperative complications of the two groups,the difference in the incidence rate of postoperative complications of different prostheses is compared,so as to obtain the difference in curative effect between cementless long-stem femoral prosthesis and cemented long-stem femoral prosthesis in the treatment of periprosthetic femoral fractures of Vancouver B2 and B3.Results: The perioperative complications of PFF are mainly include anemia,cardiovascular diseases and respiratory diseases,and good prognosis has been achieved after postive treatment.There was no significant difference in preoperative hip joint function score between the two groups(P>0.05).Harris score of the experimental group from 6 months to 1 year after operation was superior to that of the control group,with statistical significance(P<0.05).Charnley score of the experimental group was superior to that of the control group,with statistical significance(P<0.05).observing the occurrence of complications at each time node after operation,the result was that the occurrence rate of complications in the experimental group was lower than that of the control group,with statistical significance(P<0.05).Conclusion: PFF,as a more serious complication after THA,has more clinical complications.Perioperative management of patients has greater challenges.Perfecting the Multi-Disciplinary Treatment(MDT)and strengthening exchanges among disciplines are helpful to adjust the general situation of patients during perioperative period.In the surgical treatment of Vancouver type B2 and B3 periprosthetic femoral fracture,cementless long-stem femoral prosthesis has better clinical effect than cemented long-stem femoral prosthesis,and the hip joint function of patients is better recovered.
