Background:Chronic obstructive pulmonary disease(COPD)is a chronic airway disease characterized by persistent respiratory symptoms and airflow limitation.As the global prevalence increases,COPD is one of the leading causes of death worldwide And cause a high economic and social burden.Therefore,timely diagnosis,treatment,and condition assessment are very important.Objective:To compare the changes and rates of VC,FVC and FEV1 in patients with chronic obstructive pulmonary disease after bronchodilators;to explore the difference and value of different positive standards in bronchodilator tests;to explore FEV1/FVC and FEV1/VC after bronchodilator Changes and consistency.Method:From January 2019 to December 2019,203 patients with COPD who were treated in the outpatient department of respiratory medicine of the Second Affiliated Hospital of Nanchang University were selected,including 169 males and 34 females;According to the GOLD chronic obstructive pulmonary function classification,6cases were mild,66 cases were moderate,94 cases were severe,and 37 cases were extremely severe.All patients underwent basic lung function measurement and lung function after bronchodilators.The measurement items include vital capacity(VC),forced vital capacity(FVC),expiratory volume in 1 second(FEV1),FEV1/VC,FEV1/FVC,compare bronchial tubes The change value and change rate of VC,FVC and FEV1 in all patients after diastolic agent,the positive rate of bronchodila-tion under different standards,etc.SPSS 24.0 software was used for statistical analysis of medical records.Result:1)After bronchodilator,VC increased from(1.92±0.69)L to(2.01±0.69)L,(t=7.412,P<0.05),FVC increased from(1.90±0.69)L to(2.00±0.69)L,(t=-8.491,P<0.05),FEV1 increased from(1.05±0.51)L to(1.12±0.51)L,(t=9.708,P<0.05),the difference was statistically significant.2)VC change value(95.07±182.75)ml after bronchodilator;FVC change value(105.61±177.21)ml,FEV1 change value(63.94±93.84)ml,there is a significant difference between the VC change value and FEV1 change value(t=2.662,P<0.05),the difference was statistically significant;there was a significant difference between the changed value of FVC and the changed value of FEV1(t=3.632,P<0.05),the difference was statistically significant.3)The change rate of VC after bronchodilator(6.28±11.45),change rate of FVC(6.85±10.72),change rate of FEV1(6.92±9.51),there is no significant difference in change rate of the three,and there is no significant difference in change rate between groups.4)The positive rate of FVC as standard bronchodilator test(BDRFVC)is26.1%;the positive rate of FEV1 as standard bronchodilator test(BDRFEV1)is5.9%;F(?~2)=30.791,P=0.000<0.05,the difference was statistically significant;3.9%of the patients met both BDRFVC and BDRFEV1 positive criteria.5)There is no significant difference in gender,age,height,weight,BMI between BDRFVC positive patients and negative patients;BDRFVC positive patients have lower pre-FEV1%pred and post-FEV1/FVC values than negative patients.6)There is no significant difference in gender,age,height,weight,BMI between BDRFEV1 positive patients and negative patients;BDRFEV1 positive patients have higher post-FEV1/FVC values than negative patients.7)Patients with COPD have high consistency between FEV/VC and FEV1/FVC after bronchodilators(K=0.986,P<0.05),which is statistically significant.Conclusion:1)In patients with COPD,after bronchodilators,VC,FVC,and FEV1 are significantly increased compared with those before bronchodilators.VC and FVC can also be considered as one of the indexes for evaluating the condition of COPD patients;2)The positive rate of BDRFVC is significantly greater than that of BDRFEV1,and FVC as a diagnostic criterion can better reflect the reversibility of the airway of COPD patients;3)Patients with COPD have a high degree of consistency between FEV1/VC and FEV1/FVC after bronchodilators.Some patients may consider using FEV1/VC instead of FEV1/FVC to diagnose COPD.
