Study On The Status Of Human Papillomavirus Infection In Women's Reproductive Tract In A Community Of Jiujiang City And Its Influencing Factors

Objective:To explore the characteristics of human papillomavirus infection on female genital tract in community of Jiujiang City,Jiangxi Province,and to analyze the high risk factors of HPV among female population in this area,so as to provide a scientific basis for the prevention and control of HPV in female genital tract in this area in the future.Methods:From January 2016 to December 2017,cross-sectional study was conducted among female residents in some community in Jiujiang City,Jiangxi Province.All the subjects were voluntarily screened for cervical cancer.According to inclusion criteria and exclusion criteria,a total of 12675 women were included in the study.Questionnaires were sent out to investigate the basic information and past medical history of the subjects.Cervical cell specimens were collected and HPV infection and genotype were detected.Genotype distribution and influencing factors of HPV in female genital tract in this area were analyzed.Results:(1)The positive infection rate of HPV was 15.93%.(2)HPV positive infection rate was the highest in 36-45 years old group,followed by 21-35 years old group.There were statistical differences in HPV positive infection rate among women in different age groups(P < 0.05).The HPV positive infection rate of those who had sex partners > 2 and had sex for the first time < 20 years old was higher than that of those who had sex partners < 2 and had sex for the first time > 20 years old(P < 0.05).The HPV positive infection rate of those who had sex partners < 20 years old was higher than that of those who had sex partners > 20 years old(P < 0.05),and the HPV positive infection rate of those with hypertension was higher than that of those without high blood pressure(P < 0.05).The positive rate of HPV in those who had smoking history,family history of cervical cancer,history of reproductive system,family history of reproductive system infection wassignificantly higher than that in those who had no smoking history,no family history of cervical cancer,no history of reproductive system disease and no infection of reproductive system(P < 0.05).The positive rate of HPV in those who had birth times > 2 and pregnancy times > 4 times was significantly higher than that in those who had birth times ?2 and pregnancy times ?4 times(P < 0.05).(3)Logistic regression analysis showed that the reproductive system infection,family history of cervical cancer,age of first sexual life ? 20 years old,times of pregnancy > 4,times of delivery > 2,number of sexual partners > 2,past smoking history and history of reproductive system diseases 8 factors were the high risk factors for female reproductive tract HPV infection(P < 0.05).Conclusions:(1)The positive infection rate of HPV in this area is close to that in most parts of China.(2)Women in the 36-45 age group in this area are susceptible to HPV infection.The high risk factors of female reproductive tract HPV infection in this area are mainly accompanied by reproductive system infection,family history of cervical cancer,history of reproductive system diseases;and first sexual life is younger,the number of pregnant times,more births,more sexual partners,and previous smoking history will also increase the risk of HPV infection.According to these high-risk factors,it is of great public health significance to develop health education measures and comprehensive prevention and control strategies for preventing HPV infection.
