Objective:To investigate the effect of driving pressure-guided lung protective ventilation on inflammatory response to thoracoscopy surgery in elderly patients.Methods:Sixty elderly patients undergoing thoracoscopic lobectomy under general anesthesia were randomly divided into the traditional lung protection ventilation group(PV group)and the driving pressure-guided ventilation group(DP group).The traditional lung protection ventilation group adopted the traditional lung protection ventilation strategy during the perioperative period,and the driving pressure-guided the lung protection ventilation group adopted the individualized minimum driving pressure during one-lung ventilation.The inflammatory factors(IL-6,TNF-α),postoperative recovery conditions(extubation time,time to get out of bed,chest tube indwelling time,postoperative hospital stay)and the incidence of pulmonary complications were compared between the two groups of patients.Results:Compared with T0,the concentrations of IL-6 and TNF-αat T1-T3 were increased in both groups(P<0.05);Compared with the PV group,the concentrations of IL-6 and TNF-αin the DP group was decreased at T1-T3(P<0.05);There were no significant differences in extubation time,out of bed activity time,chest tube indwelling time,postoperative hospital stay,and the incidence of postoperative pulmonary complications between the two groups.Conclusion:Driving pressure-guided lung protection ventilation can reduce inflammatory response in elderly patients undergoing thoracoscopy.
