Objective:Use an ultrasound probe at different angles to the skin to puncture and catheterization the internal jugular vein and observe the effect.Metmods:A total of 120 patients who needed to undergo internal jugular vein catheterization were selected as the research subjects.Randomly divided into four groups A,B,C,D,according to the angle formed by the ultrasound probe and the skin,group A(90 °),group B(60 °),group C(45 °),group D(30 °)30 cases in each group.During the puncture,the puncture needle should be kept relatively fixed to the position of the ultrasound probe.The anatomical changes of the internal jugular vein,the effect of internal jugular vein puncture and the effects of complications were compared between the four groups of ultrasound probes and the skin at different angles.Results:Comparison of internal jugular vein anatomy between four groups: area of right internal jugular vein,shortest distance of skin from right internal jugular vein,longest distance of right internal jugular vein from skin,depth of center of right internal jugular vein A,B,C,D The difference was statistically significant(F = 19.782,5.314,18.260,4.319,P <0.05).Comparison of the effect of internal jugular vein puncture and catheterization between the four groups: duration of puncture,total length of time to complete puncture and catheterization,number of successful one-time cases,total number of puncture cases,number of cases that did not penetrate the inner wall,and number of successful cases of catheterization,The number of successful one-time catheter placements was statistically significant between groups A,B,C,and D(F /?2 = 17.38,13.606,21.23,8.92,17.50,11.79,10.601,P <0.05),of which puncture Duration,total length of time to complete the puncture catheterization,and thenumber of successful one-time puncture groups B and C decreased compared with groups A and D(P <0.05),the total number of puncture cases decreased in group C compared to group D(P <0.05),penetrated the inner wall The number of cases in groups C and D was lower than that in groups A and B(P <0.05).The number of successful cases of one-time insertion of guidewire and the number of successful cases of one-time catheter placement were lower in groups A than in groups B,C,and D(P <0.05).Comparison of the occurrence of complications between the four groups:the occurrence of complications was statistically significant between groups A,B,C,and D(F = 8.67,P = 0.031),and the incidence of complications in groups B and C Compared with group A and D,it was decreased(P <0.05).Conclusion:Although the angle between the ultrasound probe and the skin was changed,the internal jugular vein puncture under ultrasound can be satisfied and satisfactory results were obtained.When the angle between the ultrasound probe and the skin was45 °,the internal jugular vein was punctured and the catheter was inserted.The rate is small,more suitable for internal jugular vein puncture catheterization.
